NOTIFICATION.� The Applicant ehall be responsible for obtaining perrnits from any af�Cctcd muni�ipality or
the Minnesota Department of Transporlation. Thc Applicant shall notify aIl affectcd emagcncy personnel,
public transit officials�local residcnts or any other affected goup. The ApPlicant shall utilize the"Gopher Staze
One Call"cxcavation notice system as required under Minnesota Statuc Chapter 2I6D,48 h�urs prior ta

SURYEY.� The installations shall be surveyed by the applicant to ineure correct plaer,rnent within Hcnncpin
County right�f-way. The applicant is roquirod to pt�cscrvc all cxisting survey monuments. If Hcnncpin County
dcterrnines survey monuments have been distiubod during construction activities,the applicarn shall accept full
responsibility for all costs incurred to re-establish these monume.nts.
7'RAFFIC CONT'RQL:The applicant shall fiunish,install,and maintair`►alt reqcured traffic contro]devices
according to tho MnMUTCD "Tennporaiy Tr�c Control Zone Layouts Field 1�anuz�I"dated April 1995,
Minnesota Manual of Uniform TrafTic Contml Devicos,dated 1991 and all current updates,the 1VZnDOT
Standard Spocifications for Construction,dated 1995 and tht N,(nDOT Traffic En�ineering Manuals Parts 1�; .
dated Janttm'y 1996 Sn�d all current updatcs. Dctour routing shall roquire a minimum 5 wor]cin�days advance
noticc to Hennepin County.
7'RBE REMOYAL:No tree or substantial shrub shall be removed from Hennepin County existing or dedicated
right-of-way unless written permission is obtainal. 'rhis permit will not serve as permission to remove
�egetation from County property. The permittee will be responsible for obtaining written permission from any
ptoperty owner prior w removing any vegetation. All plants or�ees removed or damaged shall bc rcplaccd in
kind by tho applicant.
TRENCHING;
• Trenches for all utilities shall be Iocated as appraved by Hennepin County. Tranches,loeated parallel with
the county road,shall not be left openod for a distanee greater chan 300 feet.
• Storage of excavated materials shall bo placed or storcd on tt�c sidc of thc operatian fazthest from thc
asveled roadway artd iu�adequato distance from the edge of the excavation to prevent tccnch collapse. The
matcrial shall not imped�any cxisting drainagc.
• A�ter the instaliation has been complc;tcd,the excavation may be backfilled with th�ori�nal excavatcd
material unlcss it consists of peat,rnuck,soft or unstablc aoil,boulders vr larga rock. If the oxcavatod
z�aterial is unsuitabl�,it shall be disposod of o�'county right-of-way�d rtplaccd witfi granular or suitable
clay soils as approved by H�nnepin County.
• All trcnchir�shail bc backfillod by the"Quality Compaction Method"as defined in Specification 2]05.3F2
of the 1995 editivn o£the 1v1��OT Standard Specificarions for Construction.
• The backfiil, compaction, and restoration shail oCcur imuiodiately after the installation of thc undcrground
faciliry.

GR4 YEL AGGR.EGATE SURFACING:Aggregatc base shall me�t"Class 2"requirements as stated within
Table 3138-1 of the 1995 Bdition of MnDOT Staudard Specifications.
DIRECTIONAL BQRING: Al!hard suxfaced roadways and cntranccs shall bc jacked or borod. All bor�d pipe
installed papeadicular to county roadways, larg�r than 4"dituneter,shrill bc pullcd thru a lar�cr diamctcr
ca►rier pipe. The internal pipe shall be adcquately supported throughout the eatire length of the carrier pipe.
The end openings shall be gcnutod andJor brickod shut. Prior to pulling,the fused pipe shall not impede any
walkway or roadway facilities. The cutter head shall be sizcd as elose as poss'ble to the diameter o;f the outer
pipe to avoid excessive voids sunownding t'�e bored hole. The borcd hole ahall be reinfarced with a bentonito
slurry. The contractor shall limit the amotmt of slurry utilized to perform th�boring operation. Al�exccssivc
slurry shall be vacuumed from tho bo�ro pits. Any slurry that cxtrud�s through thc pavcment or sidevvalk
surfaccs shall immediately be removed.
JrlCKING: A casing pipe is required on all jacking insta)lations,unless a certified MnDOT"jacking",
reinforced concrete pipe is utilized for stonn drainage applications. MnDOT Spocification No.2501.3C1 shall
govern placemem. The augct shall not cxtend subataatially beyond the casing pipe. Th�casing shall be fillcd
with sand or pea rock and the ends shall be grouted and/or bricked shut
BORE/JACKING PITS: The pit shall be locabed a minimum of 5 feet from the back of curb in au urbt�n aroa or
at least 10 feet from the neareat c�ige of thc tr�volcd lane in a rural arca. All pits shall bc protcct¢d wich cight
onm�c"snow"fence,flashers,barrcls,Jersey barrier,sheeting,cribbing.ctc.or as rcquired by Hennepin
County. Thc contractor shall take every precaution feasible to protect their open excavations at all times.
Whcnever possible,the excavation shall be back�lled and properly compaeted at the canclusion af lh.e work
shifl. No excavation shall be made in advance Qf the actual work within the e�ccavation.

emergencics. �
• Hennepin Couniy Permit staffshall determine the pavement 4pening dimensions.
• T'ho pavement shall be sawed or milled in a ntat,straight vatical linc. Thc sidcs of the pavcmcnt shall be
smooib�to insurc a uniform bond to thc ncw patch.
• A bituminous tack coat mecting the roquirements of MnDOT Specification No. 2357 shall be applied to all
bitnminous or concrete adjacent surfaccs prior to placing bituminous mix. If an emulsifiod tack coal is
ntilized,adec�uate time shall be given to ensure tho"brcalcdown"of the tack coat prior to mix plaeement.
• Bituminous surfaces shall be rcplaced with rnix conforming to MnDOT Spccification No. 2331 Type
«�7�n.
• Thc contractor sha11 remove and deliver all bituminous pavements to a bituminous rccycling facitity.
• Any pavement that has bevn scarred by equipmeat, lugs,cleats,tt.tth,etc. shall bc repaired or rcplaced as
directed by�Icnnepin County,
CONCRETE PAVEMENT:
• No openfng shzll be made ia concrete pavements during winter mo�aths,cxccpt for cmergencies
• The pavement shall be sawed the full dtpth of the concrcte,rcmoved.and delivered to a concretc recy�cling
facility.
• Concrete surfacts shall be replaccd with mix dcsigna.tion 3A42 High Eazly conerete eonforming to VInL10T
Specification No. 2441.
� The concrete pavement insCa)aation shall conform to MnDOT Spxification No. 2301.
