(b)whether he may reiuse or is le2ally required to supply the requested daza;(c)any lmown consequence arising from his supplying or refusu:g to supply
private or conridendal daca:and(d)the idend[y of other oersons or enanes au[horized b,v state or federal law to receive the data. This requirement shall
not apply when an individual is asked co supply invesri2anve data, pursuan[[o secdon 13.82, subdivision 5, to a law enforcement o�icer.
