Sec.13.04 RIGHTS OF SUBJECTS OF DATA
Subd. i. Type of data. 'Ihe rights of individual on whom che data is stored or to be stored shall be as set fo�th in this secdon.
Subd.2. Information reqirired to be givm individual. An individual asked w supply private or confideatial data coacecning himself shall
be informed of: (a)�the purpose and intended use of the cequesred data within the collecting�tate agency,polidcal subdivision,or statewide system;
(b)whecher he may iefuse oY is legatly mquired to supply the requested data:(c)any known coasequence arising fmm his supplying or refusing w supp(y
private or confidenrial data;and(d)the idenary of other persoos or enades au[horizcd by stau or federal law to receive the data. This requirement s6a11
not apply when an individual is asked to supply invesdgadve data,pursuanc to secdon 13.82,subdivuion 5, w a law enforcement officer.

Subd.3. Access to data by individual. Upon request to a responsible authoriry,an individual shall be informed whe[her he is the subject
of swced data on individuals,and whe[her it is class�ed as public,priva[e or confidential. Upon his further request,aa individual who is the subject
of stored private or public data on individuals shall be shown the data wirhout any charge to him and;if he desires,shall be informed of the content
and meaning of that dara. After an individual has been shown the private dara and informed of its meaning,the data need not be disclosed to him for
six months thereafrer unless a dispuu or acaon pursuanc to this secaon is pending or addiaonal data on the individual has been collected or created.
The responsible authoriry shall provide copies of the private or pubiic data upon request by the individual subject of the data. The responsible authoriry
may require the requesting person to pay the acmal cosu of making,certifying,and compiling the copies.
The responsible authoriry shall compiy immediately,if possible,with any request made pursuant w this subdivision,or within five days of
the date of the request,excluding Saturdays,Sundays and legal holidays,if immediate compliance is not possible. If he cannot comply with the request
within that_time,he shall so inform the individual,and may have an addidonal five days within which to comply with the request,excluding Saturdays,
Sundays and legal holidays.
Subd.4. Procedure when data is not accurate or complete. An mdividuat may contest the accuracy or completeness of pubiic or private
data conceming himself. To exercise chis righc,an individual shall noafy in wridng the respoasibie authoriry describing che nature of the disagreement.
The responsible authoriry shall within 30 days either: (a)correct rhe data found ro be inaccurate or incomplete and aaempc to nodfy past recipienu of
inaccurate or incomplete dara,including recipientc named by the iadividual;or(b)nodfy the individual that he believes[he data to be correcG Data
in dispute shall be disclosed only if the individual's statemenc of disagreement is included with the disclosed data.
The dearminadon of the rzsponsible au[horiry may be appealed pursuant m the provisions of the administradve procedare act reladng w
contesud cases.
