AGREEMENT AND SWORN STATEMENT
The work for which this permit is issued shall be performed according to
the approved plans and specifications,applicable City approvals,and the
State Building Code. This permit is for only the work described and dces
not grant pertnission for additional or related work which requires separate
permits. All provisions of laws and ordinances governing this type of work
shall be compied with whether or not specified herein.This permit will
expire and become null and void if cons[ruction a�thorized is not
commenced within 180 days of the date of issuance,or if construction is
suspended for a period of 180 days at any time after work has commenced.
The applicant is responsible for assuring all required inspections are
requested in confortnance with the State Building Code.This permit may be
revoked at any time for due cause.

8. MONTHLY BIWNG. As the C{ty receives consultant btlls for incurred costs,the Cit}r wIN in turn
sand a bill to the Owners. Owners ehall be responalble for peyment to the Cfty wfthln 90 days of the Ownere' '
recelpt�bill.
4. DISSURSeMEN'f FROM E8CROW ACCOUNT. in the ev�t th� the Owners do not make I
payment to the City wtthin the tlmefreme outUned in #3 ebove, shall cease aq reviews uMll the Owners pay all �
e�ensea invoiced pursuant to�3, 7he City may dtew from the esarow e�r�t wfthout further appro�i of the
Owners to relmburae the City for eligible expenses the City hae incurred.
6. CL081NG ESCROW. The Bale�e on deposit In the escrow, if any, shall be retumed to the
Owners when all requirements related to the pro)ect are compl�o. Ctty S�aff shaA review the terms af this escrow
agreemerrt two �mea per year to de�ermine whe#�er the requfremer�ts of 1he projed har� been suxessfufly
oompleted and whether it is appropriete to retum the funds. Owner may also request the release of the funds,
and such funds ahell be�upon Ctty b'tetf reoelving the spproprlate v�erlflcatlon that e1i requfrements of the ;
proJect have been succeesfulry completed.
8. C�RTIFY UNPAID CHAROE8. If the projeot is abandoned by auvners,or if the eligible expenaes
Incurred by the Cfty euaceed the amourrt in escrow,the City shall hav�the right fo certlty the unpeid balance to the
aubjed property pursuant to Minn.Stei.§§A16.01 and 366.092.
