I was reviewing the options, and am considering recommending the following sequence: 1) the removal of the loose 'fall hazard' material from the bottom of the block slab 2) the installation of the concrete formwork from the bottom of the block slab including plywood and construction grade dimensional lumber shoring 3) removal of the material strip from the top of the slab 4) setting rebar and 5) casting concrete and finishing using the formwork installed prior to material removal. This would be a bit slower than 'dropping' the material, however it would remove shoring during demolition and allow a more controlled demolition process. Note that the patching of the miscellaneous small areas would be done from the bottom of the slab. Is this procedure acceptable to you? Grant, PE, LEED AP Structural Engineer 1286 Oldridge Avenue N West Lakeland, MN 55082 Date: April 20, 2012 To: Ross Langhans Structures Unlimited, Inc Maple Plain, MN 55359 Project#: 12025 Subject: Garage Repair 1180 Loma Linda Lane Orono, MN Dear Ross: As requested, I have performed a structural review of the proposed garage floor repair at 1180 Loma Linda Lane in Orono, MN. The scope of my work included one site visit performed on April 14,2012 where the existing garage supported floor structure was observed at the proposed work area. My calculations indicate that the new framing members and repairs as shown on the attached Structural Notes and sheets S1 through S8 dated 20APR2012 are in accordance with the structural provisions of the current Minnesota State Building Code. The opinions and recommendations contained in this submittal are based on design-check calculations performed based on the information gathered, on information povided by you, the Owner, and a site visit performed as a part of this project. It is possible that conditions exist that cannot be discovered or judged as a result of the limited nature of the site visit; if field conditions are discovered during construction that affect these plans contact the structural engineer for further discussion. The work provided in the preparation of the report concerns the structural system only and is not intended to address mechanical, electrical or plumbing systems, fire protection or accessibility. This report does not address any portion of the structure other than those areas mentioned. It does not provide any warranty, either expressed or implied, for any portion of the structure. In recognition of the relative risks, rewards and benefits of the project, the risks have been allocated so that the Cliertt agrees that, to the fullest extent permitted by law, Arlen P. Grant's total liability to the Client shall not exceed four times the total amount paid for the structural engineering services. If you have any questions, please feel free to contact me. Sincerely, Arlen P. Grant, PE, LEED AP I hereby certify that this plan, specification, or report was prepared by me or under my direct supervision and that I am a duly Licensed Professional Engineer under the laws of the State of Minnesota. TEMPORARY BRACING Contractor is responsible for bracing, without overstressing, all structural elements as required at all stages of construction until completion of this project. Use caution when operating equipment adjacent to foundation walls. DIMENSION LUMBER All lumber shall be of good quality and not have large splits and checks and shall be visually inspected by the contractor at the time of construction. POURED CONCRETE Concrete shall not be laid when the temperatures of the outside air is below 40°F, unless approved methods are used during construction to prevent damage to the concrete. OPENINGS Verify size and location of all openings with architectural and mechanical. Openings in floor and roof not shown on structural drawings must be placed between structural members. Cutting and notching which exceeds the limits of the information provided on the structural drawings must be approved by the structural engineer. SUGGESTED PROCEDURE The following contains the major steps of a basic procedure that was coordinated with the General Contractor regarding the 64"wide strip replacement. Alternate procedures are acceptable provided they do not overstress the structure during construction and result in the same finished condition. 1) Remove the loose material from the bottom of the supported masonry block slab. 2) Install the concrete formwork from the bottom of the block slab including plywood and dimensional lumber shoring. 3) Sawcut and remove of the 64"wide strip working from the top of the sfab. 4) Finish installing formwork and then install the waterstop and rebar. Take photos and email to structural engineer for review prior to casting concrete. 5) Touch up epoxy coated rebar with spray paint and coat the forms with form release agent. Do not coat epoxy coated rebar with form release agent. 6) Cast concrete using proper concrete vibration procedures and finish to match existing slab. 7) After concrete has cured, remove formwork, grind fins, and complete patching operations. 20APR2012 PROJECT: 12025 Page 1 of 1 Azv, S W J Y J 240± FIELD VERIFY a FOR MATERIALS 3 w`"P° Y V A Z Q 1W - - - - - - - - - - - - W II F-U EXISTING MAS❑NRY I WZ W !
