This permit is mot transferabte without Dis#rict approval, and is valid to the date of expirati n. No
activity is auth�rized beyond the expiration date. If the permittee requires more time to coknplete
the project, an�appiication for renewal of the permit must be received b�the Distric#at Ieaist 30
days before e piration. ,
The applicant is responsible for eompliance with all Districf Rules and for#he action of th ir
representatives, contractors, and employees.
Conditions: �'roject to be completed a� descri�ed in plans submitted to the
'MCWD offiee on May 25, 2016 according to the provisions of thi� permit.
• Properly install and maintain all required erosion control measures unti the
dis#urbed ar,eas are re-stabilized
• Notify MCWD in writing upon completing installation of perimeter and
sedimentation control�
• When the site is re-stabilized and the MCWD staff has performed a fin I
inspection, all perimeter control must be removed
