Type of data. The rights of individuai on whom the data is stored or to be stored shall be as set forth in this section. Subd.2. Infonnation required to be given individual.An individual asked to supply private orconfidential data conceming himselfshall be informed of: (a)the purpose and intended use of die requested data within the collecting state agency,political subdivision,or statewide system;(b) whether he may refuse or is legally required to supply the requested data;(c)any known consequence arising from his supplying or refusing to supply private or confidential data;and(d)the identiry of other persons or entities authorized by stste or federal law to receive the data.This requirement shall not apply when an individual is asked to supply investigative data,pursuant to section 13.82,subdivision 5,to a law enforcement officer. Remove the drywall on the remaiuder of the walls. Framing and Drywall: • Reframe the closet door to allow for the installation of a pocket door. • Add drywall backer as necessary to enable the ceilings in the closet to be raised to the bottom of the trusses. • Construct a shower bench 18"x 18"x 38"out of treated plywood to be covered by ceramic tile. • Frame the shower ceiling down to allow for the crown molding and rainwater shower head piping. • Install drywall in all walls�ld the ceiling of the shower and closet and tape ceilings and walls three coats smooth for paint. Millwork and Cabinetry: • The bathroom cabinets to have doors and drawer fronts to match the plain and fancy sample door and drawer supplied by John Lassila. • Supply and install the following cabinets: (Match height of Deromas & Husseys) • 5'3" wide x 2'10 1/2" high x 1'9" deep vanity base cabinet with no finished ends • 1'4"x 1'0 x 4'10"upper cabinet with mirror(in glazing pricing)door panel at the lefty end of the vanity. • 6'0" wide x 2'10 1/2" high x 1'9" deep vanity base cabinet with two finished ends • 2 ea. 1'4"x 1'0 x 4'10"upper cabinet with mirror(in glazing pricing)door panels at each r end of the vanity. • 4'7"wide x 8'0"high x 1'3"/1'8"deep linen storage cabinet with two doors with hardware cloth inset panels(Allowance for hardware cloth of$400.00 is included in cabinet price) and six drawers to fit into the recess between the shower and walk in closet. • Supply and install a framed mirror(in glazing pricing) at each vanity • We have included an allowance of$250.00 for the purchase of knobs and pulls for the cabinets. • Supply and install a 6 panel fir pocket door with 2 1/4" primed princeton stop and casing. • Install double 1 5/16" chrome rod and melamine shelves in the closet on 4' wall. • Supply and install a shoe rack 1'4" wide x 10 shelves high on the north wall of the closet. • Install3 %2" primed crown molding at the ceiling of the bathroom and around the cabinets. • Install customers bathroom accessories. • Cambria in the Coswell Creme color in the following areas: • 1-vanity top 22"x 63"with two side splashes and a back splash and an OG front edge, one sink cut out. • 1-vanity top 22"x 72"with a partial back splash,an OG edge,and one sink cut out. Electrical: Demo:make up light over both vanities.Both switches can be reused for new lighting at the vanities. • switch for existing shower to be used to power a recessed can in ceiling vs in soffit of the shower(the shower light to be moved from soffited ceiling of shower to ceiling of closet) • Move GFI on side wall of Gary's vanity where the pocket door is going • Possibly move outlet at Mary's vanity due to upper cabinet New: • Add 2- sconce lights at the side of the upper cabinets at Mary's vanity supplied by customer(may need wiremold inside upper cabinet) • Add a recessed can at the center of the vanity connected to two recessed cans over Gary's vanity. • Add recessed can over new shower.(the fixture will be offset to allow for the rainwater shower head) • Replace the existing bath fan with a new 70 cfm Panasonic bath fan. • Add drawer outlet for hair dryer. Plumbing: • Demo existing shower and tub valves and cap off waste,vent,and water supplies below the floor or behind the walls. • Rough in the new shower drain,valves,and heads.(The rainwater head in the ceiling will require the furring down of the shower ceiling slightly to prevent the water from freezing in the winter) • Furnish and install the following plumbing fixtures and valves: • 1-Kohler K304K valve,KT396 brushed nickel trim,with a K672 diverter valve and a KT376-4 brushed nickel trim,with a ceiling pipe and flange with K10122 CP(available � in chrome only) shower head. • Install a 2" vinyl pan drain for ceramic shower. • 2-K23 50 biscuit Devonshire under mount lavatories with K394-4 Devonshire brushed nickel wide spread lavatory faucets. • 1-K3457 biscuit Devonshire elongated water closet with K4684 biscuit seat and a K484 brushed nickel trip lever. Ceramic Tile: • Floor: Install a mudset substrate on the floor of the bathroom and shower. • Supply and insta1112x 12 Katelo AV 143-12 x 12 with ceramic base on the main floor and grout it. • Supply and install Katelo AV 143- 2x2 tile and grout it. • Shower walls:Katelo AV 143-6x 12 over concrete board to 8'0"ceiling, 1-recessed shampoo compartment with a shelf(21eve1),soap shelf,.Also includes a bench on the one end of the shower to be constructed from treated plywood and covered with concrete board and waterproofed,then set on Katelo AV 143 6x 12 ceramic tile. The tile will be grouted. Glazing: • 1-shower wa1U door brushed nickel frameless clear glass 5'0"wide with a 2'0"door and a header to support it. • 1- set of three mirrors for the 6'0"vanity(two for doors, one for center mirror). • 1- set of two mirrors for the 5'3" vanity(one for the door, one for the center mirror). Laundry Room: Demolition/framing/drywall: • Remove the existing cabinet,countertops,ceramic tile floor in the laundry and closet,soffits,and remove the drywall necessary to make electrical changes. • frame opening for the Iron-a-Way • Remove the wallpaper. • Fill in the drywall at remodeled areas and tape three coats, and sand smooth for paint. Millwork and trim: • Supply and install the following cabinets:All laundry room cabinets to be paint grade with full overlay flat panel doors, melamine interiors. • 1- 4'0"w x 36"h x 24"d sink base cabinet • 1- 4'0"w x 36"h x 24"d base cabinet with drawers and doors. • 1- 8'6"w x 42"h x 12"d upper cabinet with doors. • 1- 4'0"w x 42"h x 12"d upper cabinet with doors. • 1-3'9"w x 72"h x 18"d locker cabinet with open bench,and beaded porch board back and a shelf for hats and board with hooks for coats.. • 1- 3'9"w x 24"h x 12" d upper locker cabinet. Plumbing: • Disconnect the laundry sink and faucet and set aside. • Reconnect the laundry sink in a slightly different location and reposition the supply and trap for the washer.
