� Sec.13.04 RIGHTS OF SUBJECTS OF DATA �
Subd.1. Type of data. The rights of indlvidual on whom the data is stored or to be stored shall be as set forth in this section.
Subd.2. Intormation required to be given individual. An individual asked to supply private or contidential data concerning himself shall be
informed of: (a)the purpose and intended use of the requested data within the collecting state agency,potiticai subdivision,or statewide system;(b)
whether he may refuse or is legatly required to supply the requested data;{c)any known consequence arising from his supplying or refusing to supply
private or confidential data;and(d)the identity of other persons or entities authorized by state or federal lavv to receive the datn. This requirement shall
not apply when an individual is asked to supply investigative data,pursuant to section 13.82,subdivision 5,to a law enforcement offlicer.
The commissioner of revenue mav alace the notice required under this subdivision in the individual income tux or orooertv tax refund
instructions instead of on those forms.
Subd.3. Access to data by individual. Upon request to a responsibie authority,an individual shall be informed whether he is the subJect of
stored data on individuals,and whether it is classi�ed as publlc,private or confldential. Upon his further request,an individual who is the subJect of
stored private or public data on individuals shall be shown the data without any charge to him and,If he desires,shall be informed of the content and
meaning of that data. After an individual has been shown the private data and Informed of its meaning,the data need not be disclosed to him for six
months thereafker unless a dispute or action pursuant to this section is pending or additional data on the individual has been coilected or created. The
responsible author(ty shull provide coptes of the private or public data upon request by the individual subject of the data. The responsible authority muy
require the requesting person to pay the actual costs of making,certifying,and compiling the copies.
The responsible authority shall compiy immediately,if possible,with any request made pursuant to this subdivision,or within five days of the _

responsible suthority shall within 30 days either: (a)correct the data found to be inaccurate or incomplete and attempt to notify past recipients of
inaccurate or incomplete data,including recipiehts named by the individual;or(b)notify the indivldual that he bclieves the data to be correcG Data in
dispute shall be disclosed only if the individual's statement of disagreement is included with the disclosed dsta.
The determination of the responsible authority may be appealed pursuunt to the provisions of the administrative procedure act relating to
contested cases.

This proposal covers the remodel of the existing bath per the following specifications and the drawings by
Susan Palmquist of Sawhill:
*Demo
-Remove and dispose of the following:
*All floor tile and concrete down to sub floor.
*70%of drywall at walls and ceiling as necessary.
*Existing cabinets, vanity top,toilet, shower enclosure, lavs and lav faucets as well as
shower faucet. (Demo —shower complete).
*South wall as necessary. (Area adjacent to closet).
*Bath fan and electric as necessary for relocation.
*Mirrored wall. (North)
Note: Disposal(removal) of demo materials would be through window as much as
possible.
*Framing
-Reframe north wall(vanity wall)to allow for the installation of 3 side by side mirrored
medicine cabinets.
Note: We may have to modify the upper left hand corner of the left medicine cabinet.
(Interior box modification).
-Reframe shower wa11s(half walls), soffit and linen closet.
Note: Linen cabinet should be reframed to minimize loss of storage space in closet.
Frame in a bench at shower. See drawing.
*Drywall
-Sheetrock and retape entire bath as necessary. Soffit, new and existing walls and closet
walls and ceiling as necessary.

-Supply and set 12"X 12"floor tile to entire bath floor.

-Grout new tile and seal grout.
-Install 4" base of same tile to perimeter.
2. Shower Floor
-Pour a new'/,minimum floated floor with rubber pan to shower floor.
-Supply and set 2" S 2"tile to the shower floor and bench. (Tile: Rubble#DOS).
-Grout new tile and seal grout.
3. Shower Walls
-Supply and install dura-rock tile backer to all walls and ceiling at shower.
-Supply and install 3/4"mosaic tile to ceiling and walls. (Tile: Kate-Lo glass mosaic
tile "TD-229).
-Tile entire north wall around medicine cabinets form top of vanity to ceiling.
-Grout new tile and seal grout.
*Millwork
-Re use existing as possible. (Door, door t�im, window, window trim). Suppiy and
install new as necessary. Tnstall an access panel ta the hallway, finish to match

-Supply and install mirrored medicine cabinets per drawing. (Supplied by Sawhill).
-Supply and install a nazrow framed glass enclosure with clear glass and polished chrome
finish to new shower. Install door to match. See drawing. See altemates for frameless
glass enclosure. Note: At this time I have not figured for recessed�dicine cabinets.
This proposal of 9-14-04 still indicates mirrored �lass walls wrth light eut outs.
*Decorating
-Prepare,prime and paint ceilings and walls(2 firush coats).
-Prepare and apply a fresh coat of finish to the bath window, door and trim.
-Prepare and refinish areas of closet and hallway affected by remodel.
*Misc.
-Install hardware and backing for the same.

-Vent fan through soffrt to existing roof cap.

-After the existing waste and verrts with AVS piping and type M copper water lines as
needed to accommodate the following: Grabow Plumbing Inc. will supply and install the
following faucets and fixtures as requested. Re rough in toilet bend—closer to rear wall.

2. Two Kohler Ca�cton undermaunt lav basins which measure 17 X 14 in white finish.

3. Two Kohler Stillness widespread lav fsucets with lever handles in chrome finish

4. Proper 2 inch waste with clamp down strainer as needed to accommodate tile shower
with pan supplied by others.
5. Kohler Stillness pressure balance valve with trim in polished chrome finish. #K-304
K/KT950-4-CP.
6. Grohe Freehander shower system with concealed fitting in chrome finish.

7. Steamist unit for above shower with digital programmable time and temperature
control in polished chrome finish #SM-79/DSC-425-10.

-Rewire bath per new pan including the following:
*Wiring for the new steam unit.

l. Frameless shower glass
Original estimate was based on use of a nazrow framed shower enclosure. The plan
of 9-14-04 specifies frameless. If we go to a full frameless enclosure the added cost
would be the following:

If the water lines to the ba.throom are plastic and only'/z",we will then need to supply
and install full size 3/4"copper from the basement up to this bathroom area. This
would also require some ceiling, floor and or wall repairs in other rooms. This would
be determined upon demolition of the existing bath.
