AGREEMENT AIYD SWORN STATEMENT
The work for which this permit is issued shall be performed according to
the approved plans and specifications,applicable City approvals,and the
State Building Code. This permit is for only the work described and does
not gran[permission for additional or related work which requires separate
permits. All provisions of laws and ordinances governing this type of work
shall be compied with whether or not specified herein.This permit will
expire and become null and void if construction authorized is not
commenced within l80 days of[he date of issuance,or if construction is
suspended for a period of 180 days at any time after work has commenced.
The applicant is responsible for assuring all required inspections are
requested in conformance with the State Building Code.This permit may be ���
revoked at any time for due cause. i�

Attached is the 1998 memo that documents the exclusion of the property at 1135 Heritage Lane from the Shoreline/Heritage
sewer project. As a result,the property was never assessed for the sewer.
Upon connection now,the Connection Charge for this project per the 2015 Orono Fee Schedule is $22,440.00 which must be
paid prior to or at the time of issuance of the sewer connection permit.
Also due at the time the permit is issued is the Metropolitan Council Environmental Services Sewer Availability Charge(SAC)in
the amount of$2,485.00 for 2015. This is paid to the City for pass-through to Metropolitan Council.
I've attached a copy of the as-built sewer map which may be helpful to you in obtaining bids from plumbers on making the actual
connection.
