The nghts o£individual on whom the data is stored or to be stored shall be as set forth in this section. Subd.2. Information required to be given individual. An individual asked to supply private or confidential data conceming himselfshall be informed of: (a)the purpose and intended use of the requested daca within the collecting scate agency,political subdivision,or statewide system;(o) whether he may refuse or is legally required to supply the requested data;(c)any kno�vn consequence arising from his supplying or refusing to supply priva[e or confidential data;and(d)the identity of other persons or entities authorized by state or federal law to receive the data.This requirement shall not apply when an individual is asked to supply investigative data,pursuanc to section 13.32,subdivision 5,to a law enforcemen[officer. The commissioner of revenue may place the notice required under this subdivision in the individual income tax or propertv tax refimd instructions instead of on those forms. Subd.3. Access to data by individuaL Upon request to a responsible authority,an individual shall be informed whether he is the subject of stored data on individuals,and whether it is classified as public,private or confidentiat. Upon his further request,an individual who is the subject of stored private or public da[a on individuals shall be shown the data without any charge to him and,if he desires,shall be infonned of the content and meaning of[hat data. After an individual has becn shown the pnvate data and informed of its meaning,the data need not be disclosed to him for six months thereafter unless a dispute or action pursuant to this section is pending or additional data on the individual has been collected or created. The responsible authority shall provide copies of the private or public data upon requcst by the individual subject of the data. The responsible authority may require the requesting person to pay the actual costs of making,ceRifying,and compiling the copies. The responsible authority shall comply immediately,if possible,with any request made pursuant to this subdivision,or wi[hin five days of [he date of the request,excluding Saturdays,Sundays and legal holidays,if immediate compliance is not possible. If he cannot comply with the request within that time,he shall so inform the individual,and may have an additional five days witltin which to comply with the request,excluding Sahtrdays, Sundays and legal holidays. Subd.4. Procedure when data is not accura[e or complete. An individual may contest the accuracy or completeness of public or private data concerning himself. To exercise this right,an individual shall notify in wiiting the responsible authoiity describing the nahtre of the disagreemenc.The responsible au[hority shall within 30 days either: (a)con�ec[the data found[o be inlccura[e or incomplete and attempt to notify past recipients o(' inaccurate or incomplete data,including recipients named by the individual;or(b)notify the individual that he believes the data to be con e: ', �+i� dispute shall be disclosed only if the individual's statement of disagreement is included with the disclosed data. The determination of the responsible authority may be appealed pursuant to the provisions of the administrative procedure act relating to contested cases. All observed discrepancies shall be reported immediately. Location, dimensions and details of recesses, depressions, openings and equipment supports shall be verified by reference to architectural, electrical and mechanical drawings. EXISTING CONDITIONS Contractor shall verify all dimensions, elevations, and details of existing structure where they affect this construction. Notify architect and engineer if there are any deviations from the contract documents. Contractor shall field verify dimensions and elevations prior to fabrication of structural members. TEMPORARY BRACING All structural members are designed for in-place loads. Contractor is responsible for bracing, without overstressing, all structural elements as required at all stages of construction until completion of this project. Provide temporary lateral support for all walls until walls are adequately braced by floor and roof structure. Provide required temporary bracing for structural steel until permanent bracing and walls are in place. OPENINGS Verify size and location of all openings with architectural and mechanical drawings. Openings in floor and roof not shown on structural drawings must be placed between structural members. The contractor shall provide sleeves through concrete slabs, joists and beams for all plumbing. Spread reinforcing as required to provide concrete cover for reinforcing. GENERAL SOIL NOTES The structure has been designed using an assumed allowable soil bearing pressure of 2000 psf on virgin soil or compacted granular fill for footings. Remove all top soil, uncompacted fill or other poor soil from the construction site as determined by the soils engineer. Slope the site to drain away from the building. FOOTINGS/FOUNDATIONS Wall footings are cast-in-place concrete with continuous reinforcing placed 3" clear of bottom and 2" clear at top and sides. Provide 30 bar dia. lap at splices and full crossing lap at corners and intersections. Unless otherwise noted, wall footings are centered under walls and column footings under columns. Footings for walls not noted otherwise shall be 12" thick with a minimum projection of 4" each side with 2-#4 continuous. Maintain minimum frost depth for all exterior footings. Cast dowels in footing for foundation walls above. Dowels to be the same quantity, size and spacing as the vertical wall reinforcing. Dowels to be 30" long, extend 9" into footings uniess noted otherwise. Contractor shall be responsible to implement hot weather concrete requirements as per ACI 305 and cold weather concrete requirements as per ACI 306. Shore all foundation walls appropriately before backfilling and compacting. SLABS ON GRADE All slabs on grade, unless noted, to be 4" thick reinforced with WWF 6x6-W1.4 x W1.4 in center of slab. Control joints shall be saw cut into strip pours as soon as concrete can accept it without raveling. Construction and/or control joints will occur at a maximum of 10'-0" o.c. at exterior slabs, maximum of 16'-0" o.c. at interior slabs and shall be cleaned and sealed for curing purposes as soon as possible. Verify floor finishes and control/construction joint locations with owner and architect. All control/construction joints shall be continuous and not staggered or offset. CONCRETE COVER ON REINFORCING Footings 3" clear, bottom and sides Slab on Grade Center reinforcing in slab REINFORCED CONCRETE BLOCK WALLS See plans for location and spacing of reinforcement in walls. Place at least (1) vertical reinforcing bar in wall corners. When one bar is in a single core, place in center, unless noted otherwise. When two bars are in a single core, place one near each face. Vertical steel shall be continuous with 30 dia. lap in column piers and 48 dia. lap at all other splices unless noted. Fill block core at vertical columns with 3,000 psi grout, rodded or vibrated in place. Wall construction shall not exceed heights of 4'-0" before placement of core grout unless cleanout holes are provided at the bottom of each grout lift, then a maximum height of 8'-0" before placement of core grout. Provide W 1.7 horizontal joint reinforcing placed every 2nd course. Provide corner bar with 2'-0" legs for each horizontal bar in all bond beams at corners and intersecting walls. Hollow unit concrete masonry shall be ASTM C90 grade N Mortar and grout shall be per ASTM C-270: Type M or S for Below grade and exterior masonry; Type N for all interior above grade masonry. Provide full mortar bedded face shells and webs around all grouted cells for full bearing and prevent leakage into adjacent cells. See architectural plans for location and detail of wall control joints. MASONRY LINTELS All openings in concrete block walis not specifically indicated to have other types of lintels shall have reinforced block lintels as follows: 1. 8" deep for spans up to 8'-0". 2. 16" deep spans from 8'-0" to 12'-0". 3. 4" and 6" wide units to be reinforced with 1-#5 bottom. 4. 8", 10" and 12" wide units to be reinforced with 2-#5 bottom. Where one lintel beam is continuous over two openings, with intermediate support, use 2-#5 top and 2-#5 bottom. Extend all lintels a minimum of 8" beyond opening and fill cores below with grout full height with 1-#5 vertical reinforcing bar unless shown otherwise. All openings in brick faced or brick walls not specifically indicated to have other types of lintels shall have: L 3 1/2 x 3 1/2 x 5/16 for each nominal 4" of masonry to a maximum span of 5'-0". L 6 x 3 1/2 x 3/8 LLV for each nominal 4" of masonry to a maximum span of 8'-0". Bear 8" each end unless noted otherwise. Place and install masonry brick wall ties as described in chapter 21 of the IBC for masonry and stud back up walls. DIMENSION LUMBER Design assumes lumber is free of splits and checks and contractor will visually inspect during installation. All lumber is to be grade stamped which is to contain grading agency, mill number or name, grade of lumber, species or species grouping or combination designation, rules under which graded, where applicable, and condition of seasoning at time of manufacture. All lumber shall be seasoned to a moisture content of 19% or less, with the indication of "S-Dry" on the grade stamp. All lumber shall be protected from the elements until which time it is used in construction. Lumber grading rules and wood species shall conform to Voluntary Products Standard PS 20-70 as published by the Department of Commerce. Grading rules shall be by an agency certified by the Board of Review of the American Lumber Standards Committee. Sills and all other lumber in contact with concrete or within 18" of contacting soil, shall be pressure treated impregnated with waterborne preservative in accordance with the American Wood Preservative Bureau (AWPB) Standard LP-2. Sill plates to be bolted to foundation wall with 5/8" anchor bolts @ 4'-0" o.c. max. to extend 11" minimum into solidly grouted foundation wall. Each sill plate to have a minimum of 2 bolts with one bolt located within 12" of each end of each piece, but not less than 4"from each end. All fasteners in contact with treated lumber shall be G185 hot dipped galvanized. Minimum nailing to be in accordance with Table 2304.9.1 of IBC unless noted otherwise. Exterior walis to have 2 x 6 studs @ 16" o.c. unless noted otherwise. Interior bearing walls to have 2 x 6 studs @ 16" o.c. unless noted otherwise. tnterior non-load bearing walls to have 2 x 4 studs @ 16" o.c. unless noted otherwise. Wood lintels and headers shall have a full 3" length of bearing at each end. Wood joists shall bear the full width of supporting members (stud wall, beams, etc.) unless otherwise noted. All beams and joists not bearing on supporting members shall be framed with "USP"joist hangers or equal. Spacing of bridging for joists shall not exceed 8'-0". All plywood and OSB shall be installed per American Plywood Association standards, including using construction adhesive for fastening to floor joists. Wall sheathing shall be 1/2" CDX, Exp. 1, plywood with a 32/16 span rating. Roof sheathing shall be 5/8" CDX, Exp. 1, plywood or OSB with a 32/16 span rating. Floor sheathing shall be 3/4" CDX, Exp. 1, tongue and groove plywood or OSB with a 48/24 span rating. Fasten all roof, wall, and floor sheathing to framing members with minimum 8d nails at 6" o.c. at panel edges and at 12" o.c. at intermediate supports, unless shown otherwise. Apply all floor and roof sheathing with long dimension perpendicular to framing and stagger joints. Gypsum sheathing to be min. 1/2"fastened with 6d cooler or wallboard nails at 7" o.c. to all framing members unless noted. WOOD TRUSSES Responsibilities of the contractor, building designer, truss manufacturer, and truss designer shall follow the publication "WTCA 1-1995 Standard Responsibilities in the Design Process Involving Metal Plate Connected Wood Trusses." Truss supplier shall notify structural engineer on any proposed revisions made to the layout indicated on this plan. Verify allowable bearing locations for girder trusses with structural engineer prior to final design stage. Provide metal bearing enhancers as necessary to utilize stud columns shown on plan. All prefabricated wood trusses shall be furnished in accordance with designs prepared by a professional engineer registered in the State of Minnesota, using the design loads and span conditions indicated, including designing gable truss webs for perpendicular to face wind loads. Truss manufacturer shall provide a truss layout and certified truss drawings prior to beginning construction. Trusses shall be designed for top and bottom chord superimposed dead and live loads as indicated above. Live load deflection of roof trusses shall be limited to 1/240 of the span. Live load deflection of floor trusses shall be limited to 1/360 of the span. Design trusses for top chord bearing or bottom chord bearing as shown in drawings. Truss configuration, pitch, overhang, etc. shall be indicated on architectural drawings. Spacing of roof and/or floor trusses shall not exceed 24" o.c. Lumber for wood trusses shall be in accordance with manufacturers recommendations. Truss manufacturer to provide girder trusses, hip jacks, step-down trusses, as required and designed to support all superimposed loads. Provide hip-sets, dormers, and piggy-back trusses as required. Truss manufacturer to specify if roof sheathing applied before placing "over-framing". Provide metal framing anchors at truss bearing to mechanically fasten truss to bearing wall or supporting member as shown in details. Truss manufacturer shall provide truss to truss connection hangers. Bridging, and bracing of truss compression and tension members, shall be installed in accordance with the truss manufacturer's design and directions. No cutting, notching, or modifications of trusses will be allowed without the manufacturer's written approval.
