General Instructions:
1. You may be required to obtain other permits, i.e.: well abandonment, sewer, etc.
2. Work must not begin unless the permit card is available on the job site.
3. A 24-48 hour notice is required for all inspections. Call (952) 249-4600.

In return for issuance of said Demolition Permit, the undersigned owner hereby agrees to:
1. Submit a survey, aerial photo or sketch showing all structures on the property. Note which structures are
to be demolished.
2. Submit a survey, aerial photo or sketch showing proposed erosion control measures in accordance with
Chapter 79, Construction Site Runoff Control.
3. Submit a copy of permit approval from the Minnehaha Creek Watershed District (MCWD). The City will
not issue a demolition permit without a copy of the permit(s) from the MCWD or documentation stating
permit(s) are not required.

4. Submit a$2,500 escrow and an escrow agreement signed by the property owner(copy attached).
� 5. Keep all structure(s) enclosed and/or secured until such time as demolition is compiete.
6. Keep all demolition debris off adjoining property and/or the public rights-of way unless specific prior
approval is obtained in writing for temporary use thereof.
7. Completely remove foundation(s)from the ground.
8. Completely dispose of all demolition debris off site in accordance with all applicable PCA requirements.
9. Abandon water wells in accordance with State Health Department regulations.
10. Call for an inspection when all debris has been removed, before backfilling.
11. Within 5 working days of superstructure removal, a final inspection shall be requested. The site shall be
left clean and clear of all debris, with any excavation filled with earth level with the adjacent ground
elevation (except when such excavation is to be used as part of a new building and such new building is
actually under construction).
12. Abandon septic systems per Minnesota Rules Chapter 7080. Ail septic tanks must be pumped, crushed
and filled with native soils. An inspection is required after the tanks are pumped and before the tanks are
crushed and filled.
13. The undersigned owner shall and hereby does indemnify and hold harmless the City of Orono, its agents,
employees and assigns from and against all claims, damages, losses or expenses, including attorney
fees, against the City, its agents, employees and assigns arising out of or resulting from the demolition
described herein as performed by the property owner, his employees, agents, subcontractors or assigns.

On ]uly 9, 2015 the City received a demolition permit application to remove a principal and accessory structure
from the properties addressed 2750 Casco Point Road; on August 10, 2015 additional information was received.
Staff conducted a preliminary review based on the information provided and recommends the following items be
submitted or revised in order for your application to be considered complete and for the plan review to continue:
1. Certificate of Survey. The City received one copy of the survey dated August 6, 2015 for each property.
Please submit an additional copy of the survey for each lot(2 for each property).
2. Proof of Ownership. According to Hennepin County, Casco Ventures, LLC is not the property owner.
Please provide a copy of the deed showing proof of ownership.
3. Zoning Disclosure Form. Please have the property owner sign to acknowledge receipt of the enclosed
Zoning Disclosure Forms (2) and return the forms to me. A copy of the signed disclosure forms will be
provided at the time of demo permit issuance.
4. Escrow & Escrow Agreement. Permits involving grading and/or review by the City's engineer require
submittal of an escrow and an escrow agreement. The purpose of the escrow is to guarantee
reimbursement to the City for out-of-pocket costs incurred during the review of your plans. Additionally
this escrow will guarantee conformance with Ciry Code Chapter 79 relating to erosion control and
stormwater. The required escrow amount for this project is $2,500. The escrow agreement tied to both
PIDs is enclosed. The property owner must sign the escrow agreement and submit a single check for
$2,500. *Note: at the time of building permit issuance for each lot, separate escrows will be required.
Please be aware, if the properties are to be developed separately it will be necessary for the City to assign a new
address to the property consisting of Lots 8&9, Block 3, Casco Heights. Please advise as to your intent to develop
the properties priorto submittal of building permits so the necessary paperwork may be completed.
Lastly,your project may trigger the Minnehaha Creek Watershed District's(MCWD) permitting requirements; please
contact the MCWD directly at 952-471-0590 regarding your project. Please note, the City of Orono will not issue a
building permit without a copy of MCWD permits or documentation from the MCWD stating the proposed project
does not trigger any of their permitting requirements. The above information is required in order for the plan

Todd,
Thank you for dropping off a copy of a survey for each property. I see I only have one copy for each property...l thought
you had given me two of each. We will need another copy of each one.
The form I was looking for was the Zoning Disclosure Form. My email below states, once we understand which property
you were referring to, the Zoning Disclosure Form would be drafted. As soon as I have these forms ready, I will email
them to you for your signature.
Please note the Zoning Disclosure Form and Escrow Agreement are to be signed by the property owner. Hennepin
County does not reflect Casco Ventures, LLC as the owners. Please provide proof of ownership.
Also, referring to the below email, we also need you to contact the Minnehaha Creek Watershed District and provide us

We received a demolition permit application for 2750 Casco Point Road. Staff has conducted a preliminary review based
on the information provided and requests the following be submitted in order for your application to be considered
completed and for our review to continue:
• There are two property identification numbers(PIN) associated with 2750 Casco Point Road. Plea�e��larif�y
which PIN is associated with your project. Are you demolishing the home?garage? or both? � .
• Please submit a survey, aerial photo or sketch plan showing all structures, noting which structures are to be
demolished and showing proposed erosion control measures.
• A signed escrow agreement was submitted, but no escrow money. Prior to issuance of the demolition permit a
check for$2,500 must be received.
• Your project may trigger the Minnehaha Creek Watershed District's (MCWD) permitting requirements; please
contact the MCWD directly at 952-471-0590 regarding your project. Please note, the City of Orono will not issue
a demolition permit without a copy of MCWD permits or documentation from the MCWD stating the proposed
project does not trigger any of their permitting requirements.

• Zoning Disclosure Form. Once we know which property the demolition is associated with a Zoning Disclosure
. Form will be created. The property owner will be required to sign a Zoning Disclosure Form and return it to the
City.
