GENERAL INFORMATION
1. You may apply for plumbing permits by mail or in person at the City o�ces. Applications will be
reviewed and a permit will be issued within two working days.
2. Permit cards will be sent by retum mail after a review is completed. PERMITS ARE NOT VALID
UNTIL YOU RECEIVE A PERMIT. WORK MUST NOT BEGIN UNTIL THE PERMIT CARD IS
POSTED ON THE JOB SITE.
3. Plumbing permits may be issued ONLY to licensed plumbing contractors and to property owners
residing in the dwelling.
4. When any new construction or remodeling is involved, a separate buitding permit must be obtained.
5. All work must be done in accordance with State Code requirements.
6. All work must be inspected and air tested before it is covered. Call (952)249-4600.

" CONTRACT PRICE or JOB COST means the actual or estimated dollar amount charged for the
permitted work including materials, labor, profit, and other fixed costs. It is the amount to be charged to
the customer for the work done. If any material, equipment, labor or installations are fumished by the
owner, tenant or any other party, the reasonable market value of such items must be added to the
estimated cost or contract price for permit fee purposes. In the event that there is a dispute on the
amount of the job cost, the City may request the submission of a signed copy of the actual contract.

Dear jim:
At your request, I visited the site at the above referenced address on Wednesday, 24 August
2016,for final site observations.The structural portion of the project had been completed.
Engineer's qualifications: I am a structural engineer registered in the State of Minnesota
(registration #19033). I graduated in 1972 with a Bachelors degree in Architectural Studies
(Cum Laude) from the University of Wisconsin—Milwaukee and in 1979 with a Masters
degree in Civil Engineering (Specialization in Structures) from the University of Wisconsin—
Madison. Upon graduation, I worked for two years in the Engineered Wood Structures
Department of the U. S. Forest Products Laboratory. In 1988, following experience in other
engineering offices, I founded ARCH�STRUCTUREs with a special interest in residential wood
structures design and evaluation.
Report summary: As described in more detail in the three items below, the work had been
completed in accord with the structural documents as well as additional verbal instructions to
the contractor,and so is approved by me.
Item #1: New basement beam: Limited construction is being performed in the basement to
remove an exisring bearing wall. The contractor has replaced the wall with a (2) 1 3/4 x 9 1/2
LVL beam. The beam span is 11'-0". Both ends of the beam are to be supported by a (2) 2x4
post. One post is supported by the end of the existing wall footing. The other post is supported
by a 24" square pad footing. (See photos of new beam and pad footing.) I have checked the
beam and pad footing sizes by calculation,and th�y are approved by me.

Item#3: Second floor shower addition: Floor framing: Floor construction is mostly existing
2x10 joists, quite adequate to support the new shower and remodeled bathroom. Contractor
has drilled new holes (approximately 2 1/2" diameter) in several short span 2x10's for a new
waste pipe. (See photo directly below.) IVo repairs are needed for the existing 2x10's because
they are adequately over-designed.

One existing joist, which had been previously damaged with an oversize hole was repaired by
sistering a piece of lumber to the side. (See photo directly below.) The work which I have
described for the floor framing has been properly completed,and so is approved by me.

Item#2: Second tloor shower addition: Roof framing: Construction of the roof and end wall
for the new shed dormer to enclose the shower has been completed. The roof rafters are
supported by a new LVL beam which also supports an exisring gable end wall. (See photo
directly below.) This framing solution was verbally discussed with the contractor during my
last site visit. The end wall with a large window opening has also been properly framed. (See
photo on following page) The work has been completed in accord with the structural docu-
ments as well as my additional verbal instructions to the contractor,and so is approved by me.
