Subd. 1. Type of data. The ri¢h�s of individua!on whom�e data is s�:�or co be stored shall be as set forth in this secdon.
Subd.2. Information reqirired to be given individual. An individual asY�3 to supply privace or confidenrial dara coacerning himself shall
be informed of: (a) the purpose and incended use of the requesud dara w:�in the colleccing'stace agency,polidcal subdivisioo,or sratewide rystem;
(b)whe�her he may refuse or is legally required to supply the requested data;(c)any i�own coascquence arising from his supptying or refusing to supp(y

data conceming himself. To exercise this ri¢ht,an individual shall noafy ia wriring c=responsible authoriry dzscribing�he nacure of the disagreemeat.
The responsible auchoriry shall within 30 days eicher: (a)corrcc the dac�icund to be inaccurate or incomplere and aaempc to nodfy pasc recipiencs of
inaccurace or incompiete dara, including recipiencs named by che individL�i;or(b)coafy the individual thac he believes the data to be correct. Dac3
in dispu�e shall be disclosed only if the individual's stacemenc of disagre�Ltnt is i:c:�ded with the disclosed data.
