Sec.13.04 RIGHTS OF SUBJECTS OF DATA �,
Subd. 1. Type of data. The righu of individual on whom the data is stored or to be stored shall be as set forth in this section.
Subd.2. Information required to be given individual. An individual asked to supply private or confidential data conceming himselfshall be
informed of. (a)the purpose and intended use of the requested data within the collecting state agency,political subdivision,or statewide system;(b)
whether he may refuse or is legally required to supply the requested data;(c)anv known consequence arising from his supplying or refusin�to supply
private or confidential data:and(d)the identity of other persons or entities authorized by state or federal law to receive the data.This requirement shall
not apply when an individual is asked to supply investigative data,pursuant to section 13.82,subdivision 5,to a law enforcement officer.
The commissioner of revenue mav olace the notice reauired under this subdivision in the individual income tax or propertv tax refund
instructions mstead of on those forms.
Subd.3. Access to data by individual. Upon request to a responsible authority,an individual shall be informed whether he is the subject of
stored data on individuals,and whether it is classified as public,private or confidential. Upon his further request,an individual who is the subject of
stored private or public data on individuals shall be shown the data without any charge to him and,if he desires,shall be informed ofthe content and
meaning of that data. Afler an individual has been shown the private data and informed of its meaning,the data need not be disclosed to him for sia
months thereafrer unless a dispute or action pursuant to this section is pending or additienal data on the individual has been collected or created. The
responsible authoriry shall provide copies of the private or public data upon request by the individual subject of the data. The responsible authoriry
may require the requesting person to pay the actual costs of making certifying,and compiling the copies.
The responsible authority shall comply immediately,if possible,with any request made pursuznt to this subdivision,or within five davs of
the date of tne request,exciudmg Sa[urdays,Sundays and legal holidays,if immediate compliance�s not possible. If tie cannot comply with the request
within tha[time,he shall so inform the individual,and may have an additional five days within which to comply with the request,excludine Saturdays,
Sundays and legal holidays.
Subd.4. Procedure when data is not accurate or complete. An individual may contest the accuracy or completeness of public or private data
concerning himself. To exercise this right,an individual shall notify in writing the responsible authority describing the nature ofthe disagreement. The
responsible authority shall within 30 days either. (a)correct the data found to be inaccurate or incomplete and attempY to notify past recipients of
inaccurate or incomplete data,including recipients named by the individual;or(b)notify the individual that he bel ieves the data to be correct. Data in
dispute shall be disclosed only if the individual's statement of disagreement is included with the disclosed data.
The determination of the responsible authority may be appealed pursuant to the provisions of the administrative procedure acl relating to
contested cases.

building permits for construction in the city. Attached is a building permit application.
Please fill out the application and return it with 2 sets of building plans, hardcover
calculations and a survey showing the location of the building. A review will be

Dear City of Orono,
Here is a current survey, hardcover calculations, and all the information we could find
about the gazebo we had erected on our property. Please accept our apology that this
permit application is occurring after the fact. We were originally thinking about a
constructed building and when we realized we could not meet the height requirements set
forth by the city, and we went a different direction. Unfortunately when we looked at this
option, Master Pool and Spa instructed us erroneously that the structure they were
bringing out was considered a temporary or portable structure and would not require a
building permit. We now know of course that this is not the case. Please let me know if
there is anything else we can provide for this process and what the amount of our check
should be. The unit is available for inspection at your convenience, but please let me
know if you need the unit unlocked for any reason. Purchase price of the unit was
$13500 and site prep was about $2000. I have attached a copy of the receipt for the unit.

incomplete. The following additional information is required to complete your application:
1. An updated survey of the property showing the location and setbacks of the proposed
gazeba The survey in our files (copy enclosed) dates from ]964 and does not show the
location of the house as constructed or any hardcover.
2. Hardcover calculations, completed by the surveyor who does the survey.
3. The defined height of your house. This is measured from the ground to a point halfway
between the peak and eve of your highest roof. The defined height of the gazebo can be
no greater than the defined height of your house.
4. Plans for the gazebo.

Y All gazebo products MUST be installed on a hard and level surface.
1. SKIRTS - Lay out all the skirts. It takes two skirts for every 100" wall and one skirt
for every 50" wall. Once the skirts are laid out, take a round corner post and attach
a skirt (if the enclosure has options, such as a bay window, sitting bay window, or
alcove room, check the layout diagram to determine the position of the options).
Attach all remaining skirts.
2. WALLS - Install the walls and door assembly.
3. SQUARE THE GAZEBO - Now square the gazebo before attaching the top plate.
To square the gazebo, measure from one round corner to the opposite round
corner. Repeat the process with the other two corners, measuring as X. The
measurements should be within '/4" of each other.
➢ A good "double check" is to make sure the doors close correctly and the
windows run smoothly in their track. If the gazebo is not squared, you will have
trouble with both.
4. TOP PLATE - Once the gazebo is square, install the top plate on the walls,
starting at one corner, and screwing down the top plate per assembly instructions.
5. WALL BLOCKS - Once the top plate is secure, put the wall blocks on the top plate
(rafters will rest against these). Refer to the Assembly Instructions for block
placement. Screw down the wall blocks and you are ready for the dome box and
rafters.
➢ TIME SAVER: Do not secure rafters to wall blocks until after the roof panels
are installed, as you may need to move rafters to ease placement of panels
for "fit".

6. RAFTERS - To mount the rafters to the dome box, turn the box upside down, and
put the pins in the dome box first, leaving 3/4" of the pin exposed. Then tape the
rafters into the pins.
➢ Now flip the dome box and rafters over (slowly) and lift the box, right side up,
over the wall and rest the rafters on the wall blocks. Secure each common
rafter to the box with two screws. The roof is now ready for roof panels.
y TIME SAVER: For dome box/rafter assembly make sure all pre-drilled holes
(for marriage of rafters/dome box) in the dome box and rafters are centered to
assure easier roof panel installation with the alignment pins. If they not
centered, simply re-drill to center.
7. DOME — Install the dome before the installation of the roof panels. This will make
it easier to maneuver your ladder under the enclosure.
➢ Holes are pre-drilled. Place the dome on the gazebo. Screw the dome down
— DO NOT OVER TIGHTEN, or the dome will crack. You may want to use a
screwdriver rather than an electric screwdriver.
8. ROOF PANELS - Lay out the roof panels (lefts, rights, and centers) as they will go
on the rafters. Start the roof panel rafter screws prior to lifting the panel onto the
roof. Begin with the centers, then go to one corner and screw down the left/right
roof panel. Continue to work your way around the gazebo until all panels are
secured. Now go inside the gazebo and screw wall blocks to the rafters.
9. FINAL INSPECTION - Inspect the gazebo for scratches and abrasions — touch-up
stain is supplied. It is not necessary to do any repairs until the gazebo is
assembled as some scratches and abrasions may not be visible after completion.
10. ANCHORING - We highly recommend that gazebos be permanently anchored to
the ground. Building codes vary (city, county, state).

Roof Assembly Step 11
Place angle cut side of the first roof panel on the 1 X 4 part of the corner rafter.
Slide the roof panel tight against the dome box and common rafter. Secure with six screws
through the pre-drilled holes in the roof panel into the rafters. Repeat This process for the
remaining seven panels. Start at one corner and work in one direction.
Rafters should still be unattached for ease of installation.

of the roof panels, centered over the
rafters. Secure with three screws each.
