The righu of individual on whom the data is stored or to be stored shall be as set forth in this section. Subd.2. Information required to be given individual. M individual asked to supply private or confidential data conceming himself shall be informed of: (a)the purpose and intended use of the requested data within the collecting'state agency,political subdivision,or statewide system; (b)whether he may refuse or is legally required to supply the requested data;(c)any known consequence arising from his supplying or refusing to supply private or confidential data;and(d)the identity of other persons or endties authorized by state or federal law to receive the data. This requirement shall not apply when an individual is asked to supply investigadve data,pursuant to section 13.82, subdivision 5,to a law enforcement officer. Access to data by individual. Upon request to a responsible authority,an individual shall be informed whether he is the subject of stored data on individuals,and whether it is classified as public,private or confidential. Upon his further request,an individual who is the subject of stored private or public data on individuals shall be shown the data without any charge ro hun and, if he desires, shall be informed of the co�tent and meaning of that data. After an individual has been shown the private data and informed of its meaning,the data need not be disclosed to him for six months thereafter unless a dispute or acrion pursuant to this secdon is pending or addidonal data on the individual has been collected or created. The responsible authority shall provide copies of the private or public data upon request by the individual subject of the data. The responsible authority may require the requesting person to pay the acival costs of making, certifying,and compiling the copies. The responsible authority shall comply immediately,if possible,with any request made pursuant to this subdivision,or within five days of the date of the request,excluding Saturdays,Sundays and legal holidays,if immediate compliance is not possible. If he cannot comply with the request within that time,he shall so inform the individual,and may have an additional five days within which to comply with the request,excluding Saturdays, Sundays and legal holidays. Subd.4. Procedure when data is not accurate or complete. An individual may contest the accuracy or completeness of public or private data concerning himself. To exercise this right,an individual shall norify in writing the responsible authoriry describing the nature of the disagreement. The responsible authority shall within 30 days either: (a)correct the data found to be inaccunte or incomplete and attempt to notify past recipients of inaccurate or incomplete data, including recipients named by the individual; or(b)notify the individual that he believes the data to be correct. Data in dispute shall be disclosed only if the individual's statement of disagreement is included with the disclosed data. The determination of the responsible authority may be appealed pursuant to the provisions of the administrative procedure act relating to contested cases. Section 0090 - Su�plementary Conditions 1.1 The General Contractor shall purchase & maintain insurance indicated in General Conditions, Article 17 and shall include the following in amounts acceptable to Owner, or greater if required by law: 1. Workmen's Compensation Insurance: Statutory. - Employer's Liability Insurance: $100,000 each accident. 2. Comprehensive General Liability shall include coverages for Premises and Operations, Independent Contractors' Protective, Products and Completed Operations, and Broad Form Property Damage. - Bodily Injury Liability - Property Damage Liability - Products and Completed Operations to be maintained for 5 years after final payment - Property Damage Liability Insurance will provide X, C or U coverage as applicable 3. Contractual Liability (including Broad Form Property Damage) - Bodily Injury - Property Damage 4. Personal Injury, with Employment Exclusion deleted 5. Comprehensive Auto Liability: - Bodily Injury - Property Damage 6. Property Insurance: Builders Risk Coverage funished by Owner under Homeowner's Policy Rider. 7. The Contractor shall submit Certificates of Insurance for the review and approval of the Owner prior to commencing work. 1.2 Contractor shall supply all labor, transportation, material, apparatus, scaffolding and tools necessary for the entire and proper completion of the work. He shall install, maintain and remove all equipment needed for the proper execution of this contract, and be responsible for the safe, proper and lawful performance of his men and equipment, maintenance, and use of the same. 1.3 All work, equipment, devices and processes shall comply with applicable building and construction codes, regulatory measures of any supervisory or regulatory govermental agency and all work, equipment and materials used shall in addition conform with requirements of latest edition of all construction standards as applicable or as incorporated by reference. All work shall be performed in strict accordance with manufacturer's/supplier's recommended standards and practices and the qualifiy of workmanship shall be in keeping with highest standards of the industry. These high quality industry standards shall govern performance and execution work in all sections. 1.4 The Contractor shall provide and pay for all labor, materials, permits (including building permit), licenses, taxes, equipment, tools, water, heat, utilities, transportation & other facilities & services as necessary for the proper execution & completion of work. 1.5 The Contractor shall deliver all work free from any and all mechanics liens, claims or encumbrances of any description for all labor, equipment, materials and other items, and shall furnish satisfactory receipts or lien waivers. Contractor shall agree to indemnify and hold and protect the Owner and the premises harmless from any liability or expense by reason of any filed lien or claim. Refer to Article 9, paragraph 9.12. 1.6 The Contractor shall warrant and guarantee all materials and workmanship no less than a period of 12 months from Owner's acceptance of the entire construction as evidenced by an acceptance certificate; except where manufacturer's warranty and guarantee exceeds this time period, then Contractor's time length shall be the same. 1.7 The Owner will furnish construction financing for the project. The Contracor shall negotiate with Owner for the percent of retainage for Contractor labor and material costs on monthly billings. 2 Division 1 Gener?1_ Renuirements Section 0110 - Summary of the Work 1.1 The work includes additions & remodeling to the existing residence outlined as follows: 1. Master Bedroom & Bathrooms Addition 2. Addition to Family Room/Kitchen w/ masonry fireplace. 3. 1st Floor Remodeling 4. Lower level storage/furnace room. 5. New furnace system for addition. 6. Exterior colored concrete patio & stair; wood retaining wall system. 6. Basic work is shown in the Drawings, sheets A1 thru A5, E1 and Specification with certain items referred to only in the Drawings: the omission of express reference to any items or parts necessary for a complete installation does not relieve the Contractor of responsibility for : total completion of the work. 7. Refer to Section 0120, Alternates for areas of work to be separated from cost of Base Bid. 1.2 The contractor shall inspect the site to determine total scope of work, and is responsible for field verifying all project dimensions. The Contractor's bid shall be based on examination of proposed work, existing conditions and areas requiring demolition and removal. 1.3 The Contractor shall cooperate with the Owner in the scheduling and execution of the work and the use of the site, and he shall notify the Owner as far as possible in advance of the commencement of any work or operation which would interfere with the use of the existing building. 1.4 The Contractor shall not make any changes that add or deduct from contract price without a written change order by Owner. 1.5 The Contractor shall provide for total project clean-up in all areas of work to site and building, such as windows, yard, etc. 1.6 The Contractor shall provide termporary centering, scaffolding, platforms, stairs, guards, runways, ladders, temporary bracing and shoring, barricades, paritions, enclosures and such other temporary work as may be necessary for the execution of the work and the protection of persons and property. Remove temporary work when no longer required. 1.7 Provide temporary bracing, shoring, underpinning and support during demolition, cutting, remodeling and related new construction as necessary for the execution of the work and the protection of persons & property. 3 1.8 Provide protective coverings and enclosures necessary to prevent damage to existing work to remain. Protect temporary openings & exterior walls. 1.9 Provide dust proof temporary enclosures separating areas under demolition & remodeling from the remainder of the building. Remove temporary work when the need no longer exists. Coordinate with Owner areas to be protected and separated as work progresses. 1.10 Every effort shall be made to keep areas requiring remodeling clean as to not disturb adjacent existing areas on a daily basis. 1.11 Work includes all demolition & removal work on the site & in the existing building as is necessary to accommodate the new construction & disposal of all removed materials & equipment; including mechanical & electrical work. Patch all areas disturbed during construction for new wiring, plumbing, structure, etc. Existing construction that is to remain which is damaged by the work shall be replaced at no additional cost. Coordinate any materials & equipment to be salvaged & turned over to Owner & relocated, or removed, as per Owner's direction. 1.12 Work includes all necessary excavation, compacted fill, backfill, granular compacted base fill for concrete slabs, rough grading, finish grading, as noted in Keynotes, & topsoil with sod to restore areas disturbed by construction to original condition. Contractor to verify location of any buried utilites prior to beginning excavation or demolition work. 1.13 Excavation work shall allow footings & foundations placed on undisturbed soil. Any soil irregularities at bottom footings which are not consistent with assumed bearing capacity for footings shall be reported to Owner before proceeding further. Organic materials, debris & unused excavation material shall be removed from site. All fill & backfill shall be a granular material & compacted to 95% of Standard Proctor Density (ASTM 698). Suitable material clean & free of organic material obtained from excavation & cutting at site may be used. Footings have been designed for a maximum soil bearing pressure of 2000 PSF. It is the responsibility of the Contractor to verify this value & insure that it will not result in damaging differential settlement. Existing topsoil to be removed & respread as finish grade. 1.14 The Contractor shall schedule the work and conduct his operations to conform with the Owner's need for use of the premises. The Contractor shall cooperate with Owner in setting the construction schedule to meet this requirement and allow Owner access during the construction period. The issue of security to the building is of primary concern to the Owner during construction and the Contractor shall coordinate with Owner in the securing 4 of the building during the entire construction period. 1.15 The Contractor shal� cooperate with the Owner in the scheduling and the execution of the work and the use of the site, and make every effort to keep site clean & free of debris on daily basis so to not interfere with the use of the existing building. �ection 0115- Base Bid 1.1 The Base Bid shall include architectural; mechanical; plumbing; and electrical work as required to complete the remodeling and additions as drawn and specified, and costs shall be divided into these four separate areas of work. Include all alternates and cost allowances as separate items to Base Bid. Contractor to provide his own bid form on letterhead including all items mentioned. In addition to the above items, the Base Bid shall have separated costs according to the following areas of work. 1. Cabinets. 2. Millwork. 3. Concrete & masonry work. 4. Drywall & plaster. 5. Plumbing fixtures & faucets. 6. Colored concrete. 7. Windows & exterior doors. 8. Finish hardware. 9. Electrical Fixtures. 10. Tile work. 11. Retaining Wall. Section 0120 - Alternates 1. Alternate A (add) - The Contractor shall furnish a Performance Bond and Labor and Material Payment Bond (AIA Document A311) for the project. 2. Alternate B (add) - Refer to 1st Floor Plan, Sheet Al, & Room Finish Schedule. Provide cost for adding veneer plaster system to walls & ceiling, room 101; ceilings @ rooms 102, 103, & 104. 3. Alternate C (add) - Refer to 1st Floor Plan, Sheet A1, & Keynote #40. Provide cost for adding stamped brick pattern to colored concrete patio slab, landing & walk to pool deck. 4. Battens to remain cedar, d & better, 2 5/8" (1 x6 cut) x 3/4". 5 Section 0130 - Submittals 1.1 Submittals - Submit shop drawings in the form of one reproducible transparency & two prints of each drawing. Submit shop drawings for work under sections where fabrication or execution is critical & refer to individual section submittals; including - all cabinet work: built-ins, millwork, kitchen cabinets, special custom fabrications, shower enclosure; windows/glass doors, etc. Submit samples as requested and needed. Submit brochure cuts for items as noted including: appliances, finish hardware, light fixtures & related items, plumbing fixtures & fittings. Section 0150 - Temporary Facilities 1.1 The Contractor shall provide temporary centering, scaffolding, platforms, stairs, guards, runways, ladders, temporary bracing & shoring, barricades, partitions, enclosures, & such other temporary work as may be necessary for the execution of the work & the protection of persons and property. Remove temporary work when no longer required. Section 0195 - Key Construction Notes, Materials, Systems Keynote Symbol O 1.1 Refer to Drawings for location symbols; Contractor to determine total scope of work. This work complements & adds to specification Divisions 3 thru 16. 1. 1'-8" wide x 10" deep formed & poured concrete footing with 2 - #4's continuous, 3" from bottom, at all 12" block walls & as noted. #1A - 1'-4" wide x 10" deep with 2 - #4's @ 8" block walls & other bearing conditions as noted, otherwise same as #1. #1B - Extend footing straight & perpendicular to existing; dowel into existing footing w/ #4 dowels at 12" o.c. extending into existing footing 8" & new footing 8". #1C - 4'-0"x 7'-0"x 12" deep concrete footing w/ 4 - #5's longitudinally & 7 - #5's transverse, 3" from bottom. #1D - Step concrete footing w/ grade, verify new finish grade elevations. 2. Top of concrete block foundation wall to be at elevation to allow 8" square precast colored concrete block base to protrude 3" above finish grade (see colored concrete, #40); anchor to foundation w/ extended vertical reinf. bar; provide threaded rod from kwik bolt receptor in precast block to connect to Simpson AB66 (6") post base. #4A - Adjust top of concrete block wall heights to allow patio slab to bear on 6" CMU & top of concrete stairs to bear on portion of 12" foundation wall. #4B - Bear base of stair & landing on 8" foundation walls; #4C - Step in intermediate wall, provide 4" top course to provide bearing for stair & concrete landing. 5. Provide new rim joist for direct bearing support for new LPI floor joists; provide 2 - 2x12's anchored to existing foundation walls w/ 3/4" sleeve anchors at 12" o.c. (verify secure anchor ea. location). #5A - 2-2x10's below LPI's, anchor to concrete block chimney support w/3 - 3/4" sleeve anchors; provide beam pocket on existing chimney foundation wall to remain. 6. Where new footings meet existing, join w/ 2 - #4 dowels extending into existing footing 8" & new footing 8". 7. Provide 4" min. stabilized base of sand & gravel fill material to 95% of Standard Proctor Density (ASTM 698) below slabs as required remove all organic soil material; compact w/ manually operated vibrating plate compactors. Verify existing grade elevations for actual thickness. 8. Footings to be placed on undisturbed soil. #7A - Remove existing pool deck brick pavers; re-lay in new configuration as required over compacted backfill. 9.
