Sec13.04 RIGHTS OF SUI3JEC'I'S OF DATA
Subd 1 Type of data. The rights of individu�l on�vhom the d�ta is stored or to be stored sh<�II be as set forth in this section.
Subd-2 Infonnation required to bc givcn individual_ An individual asked to suppl}'privatcor conCidential data conceming himselfshall be
informed oC (a)the purpose and intended use of the requested data within the col lecting state agency,political subdivision,or statewide system;(b)
whether he may refuse or is legally required to supply the requested data;(c)any known consequence arising from his supplying or refusing to supply
private or confidcntial data;and(d)the identity ofother persons or entities authorized by state or federal law to receive the data I his requirement shall
not npply when an individual is asked to supply investigative data,pursuant to section 13 82,subdivision 5,to a law enforcement officer_

Please tind enclosed a bliilding application for the above mentioned address. Along with
the overall basement plan, a plan of just the remodeli►Ig area and tinally a survey of�the
existing lot showin� the existing house along with elevations.
We propose to remodel the existi�lg basement. We will upgrade the existing batliroom,
latuldry and family room. We will be upgrading the electrical and insulation; and addi�Ig
new plumbing tixtures, sheetrock, trim, paint and carpet. We will be adding 3 egress
casement windows. The size of each window is 26" wide x 36" high.
