General Instructions:
1. You mav be required to obtain other permits, i.e.: well abandonment, sewer, etc.
2. Work must not bebin unless the permit card is available on the job site.
3. A 24-48 hour notice is required far all inspections. Call (952) 249-4600.
4. Sewer must be discontinued at the City service by qualified contractor before demo permit is issued.

In return for issuance of said Demolition Permit, the undersigned owner hereby agrees to:
1. Submit a survey, aerial photo or sketch showing all structures on the property. Note which
structures are to be demolished.
2. Submit a survey, aerial photo or sketch showing proposed erosion control measures in
accordance with Chapter 79, Construction Site Runoff Control.
3. Submit a$2,000 escrow and an escrow agreement signed by the property owner.
4. Keep all structure(s) enclosed and/or secured until such time as demolition is complete.
5. Keep all demolition debris off adjoining property and/or the public rights-of way unless specific
prior approval is obtained in writing for temporary use thereof.
6. Completely remove foundation(s) from the ground.
7. Completely dispose of all demolition debris off site in accordance with all applicable PCA
requirements.
8. Abandon water wells in accordance with State Health Department regulations.
9. Call for an inspection when all debris has been removed, before backfilling.
10. Within 5 workinb days of superstructure removal, a final inspection shall be requested. The site
shall be left clean and clear of all debris, with any excavation filled with earth level with the
adjacent ground elevation (except when such excavation is to be used as part of a new building
and such new building is actually under construction).
1 l. Abandon septic systems per Minnesota Rules Chapter 7080. All septic tanks must be pumped,
crushed and filled with native soils. An inspection is required after the tanks are pumped and
before the tanks are crushed and filled.
12. The undersigned owner shall and hereby does indemnify and hold harmless the City of Orono, its
agents, employees and assi�ns from and against all claims, damages, losses or expenses,
including attorney fees, against the City, its agents, employees and assigns arising out of or
resulting from the demolition described herein as performed by the property owner, his
employees, agents, subcontractors or assigns.

Attached is the zoning disclosure form and escrow agreement pertaining to demolition permit
#2012-00552 that we spoke about. You can print the forms off and bring them in with you or
we will have a copy of each one here at the office for you to sign. We will also need a $2,000
check, made payable to the City of Orono, in conjunction with the escrow agreement.

I spoke with the people that are handling the insurance claim for the property owner. They are close to
resolving the claims. The reason the home has not been demolished is because the insurance company has
been dragging their feet on performing a final inspection. I have informed them that I will be putting this item
on the 6/11 agenda regardless of any insurance issues. To the extent staff continues to receive calls— I would
encourage you to inform the residents that this item will be on the 6/11 agenda requesting authorization to
proceed with the demolition of the building.

Hi Jean,
I spoke with Lyle. He said he has had some conversations over the year with Wendy Sullivan (owner) and
recently a call from Fretland's. Lyle said Fretland's indicated they are still working on resolving the case. Lyle
informed Fretland to sent a letter to that affect, but he informed them we were still turning this case over to our
attorney as the neighbors are very upset this has gone unresolved for so long.

RE: 325 Crestview Avenue — Hazardous Building
On July 4, 2011 the home at 325 Crestview Avenue was destroyed by fire. The owner,
Wendy Sullivan, has stated she is working with the insurance company to settle her claims.
As of this date, the remains of the home have not been removed. The City is concerned about
her ability to rebuild. An order to remove the debris was sent to Ms. Sullivan on April 26, 2012.
On April 30, 2012 the letter was returned to the City marked "Return to Sender".
Please start a hazardous building action to have the lot cleared of debris and foundation.

Dear Mr. Oman:
I am writing to give you an update of the status of the above captioned fire. We
have been retained by Ms. Sullivan to assist her in the preparation of her claim
submission to the insurance company.
We are currently in the final stage of the preparation of the personal property
inventory. We plan to have it complete and submitted by May 23, 2012. The insurance
policy states that the insured must maintain the property for inspection, until the company
agrees on the amount of loss and damage.
We are requesting on behalf of Ms. Sullivan, an extension of time for her to
demolish the dwelling until she has a final agreement with the insurance company.
