Sec.73.04 RIGIITS OF SUBJECTS OF DATA
Subd.1. Type of'data. The rig.hts of individual on whom the data is stored or to be stored shall be as set forth in this section.
Subd Z.inf'ormadon required[o be given individual.An individual asked to supply private or wnfiden[ial data concerning himself shall be
informed of: (a)the purpose and intended use oCthe requested da[a within the collecting state agency,political subdivision,or statewide system;(b)
whether he may retuse or is legally requued[o supply the requested data;(c)any known consequence arising from his supplying or refusing to supply
priva[e or confiden[ial data;and(d)the identity of other persons or enti[ies authorized by state or federal law to receive the data.This reqtirement shall
no[apply when an individual is asked to supply investigative data,pursuant to section 13.82,subdivision 5,to a law enforcement of6cer.
The comrnissioner of revenue mav vlace the notice reouired under this subdivision in the individual income tax or oroperri tax refund
instrucdons mstead of on[hose forms.
Subd.3. Access to data by individual.Upon request to a responsible authority,an individual shall be informed whe[her he is the subject of
stored data on imiividuals,and whe[her it is class�ed as public,private or wnfidential. Upon his fiirther request,an individual who is the subjec[of
stored priva[e or public data on individuals shall be shown the data without any chazge to him and,if he desires,shall be informed of[he con[ent and
meaning of that data. After an individual has been shown the private data and informed of its meaning,the data need not be disclosed to him for six
months thereafier unless a dispute or acuon pursuant to this section is pending or additional data on the individual has been collected or created. The
responsible au[horiTy shall provide copies of the private or public data upon request by the individual subject of the data. The responsible authority
may requirc the requesting person to pay the acNal costs ufmaking,certifying,and compiling the copies.
The responsible authority shall comply immediately,ifpossible,with any request made pursuan[to this subdivision,or within five days of
[he date of the request,excluding Saturdays,Sundays and legal holidays,if irnmediate compliance is not possible. Ifhe cannot comply with the reyuest
within that time,he shall so inform the individual,and may have an addi[ional fivc days wi[hin which to comply with the request,excluding Sa[urdays,
Sundays and legal holidays.
Subd.4.Procedure when data is not accurate or wmplete.An individual may contes[the accuracy or completeness of public or private data
concerning himself. To eaercise this right,an individual shall notify in writing the responsible authority describin�the nature of the disagreement.1'hc
responsible au[hority shall within 30 days either: (a)eorrect the data found to be inaccurate or incomplete and attempt to notify past recipients of
inaccurate or incomplete data,including recipients named by[he individual;or(b)notify the individual that he believes the data to be correct. Data in
dispute shall be disclosed only if the individual's statement of disagreement is included wi[h[he disclosed data.
The de[ermination of the responsible authority may be appealed pursuant to the provisions of the administrative procedure act relaling[o
contested cases.

III. Front Porch/Foyer Enlargement
Using the e�sting foundation wall (currently below porch area), we will increase the
size of the front entrance to the house. The area of expansion increases the square
footage of the 151 floor by 82.5 sq ft.

I V. Repair Garage
E�usting garage has a bowed foundation wall. Removal of existing slab and garage
foundation and stud walls wilUmay be necessary. Footings, slab, foundation wall and
stud walls will be repaired/replaced as necessary.
V. Addition of Master Bedroom
A master bedroom will be constructed above the garage and connected with the
existing 2"d floor of the house. The master bedroom will increase the 2nd floor by 397
sq. ft.
VI. New Roof Trusses
The entire main body roof and trusses of the house will be removed and will be
replaced as per architectural drafts. The new dormers in the existing 2°d floor(not
including the master bedroom will increase the 2°d floor by 118.5 sq ft.
VII. Basement
The remodeling overview in the basement includes:
■ One egress window well.
■ Replacement of 4 casement windows.
■ Addition of new basement door.
■ Stud walls and door around mechanicals.
■ One bathroom rough in.
VIII. 1 St Floor
The remodeling overview of first floor includes:
■ First floor general bathroom.

A new 12' x 24' shed/slab is proposed south of the garage. Plans will be provided.
XI. Siding and windows
■ New siding will be installed to the entire house, garage and shed.
■ Windows will be installed as per azchitectural draft.
XII. Fireplace
A wood buming fireplace will be installed on the first floor family room. A gas
fireplace will be installed on the second floor master bedroom. Permits will be applied
for before work will take place.
XIII. Sidewalks Steps
Will be planned and installed as necessary to meet or exceed Orono city codes.
XIV. Special Permits
We will apply for(or have subcontractors apply for)any permits necessary to complete
this project and look forward to working with the City of Orono.

Dear Mr. & Mrs. Bautista:
I am writing in regards to a complaint this Office has received concerning excavating
within 5 feet of the property line. The City of Orono prohibits excavating within 5 feet of
the property line due to the potential to cause changes in drainage and potential for
erosion. The City also requires a permit for excavating, grading or filling in excess of 10
cubic yards. It appears that you are beyond that limitation also.
I will not require you to apply for the land alteration permit if you restore the excavated
area within 5 feet of the property line. Please restore the excavated soil by November 7, .
2008. This Office will verify compliance after that date and if the area is found restored,
no further enforcement action will be required. If you have any questions concerning this
matter please feel free to call.
