In return for issuance of said Demolition Permit,the undersigned owner hereby agrees as follows:
1. The structure(s) shall be kept enclosed and/or secured until such time as demolition is
complete.
2. Demolition debris will be kept off adjoining property and/or the public rights-of-way unless
specific prior approval is obtained in writing for temporary use thereof.
3. Foundations shall be completely removed from the ground.
4. All demolition debris shall be completely disposed of off site in accordance with all
applicable PCA requirements.
5. Water wells must be abandoned in accordance with State Health Department regulations.
6. Inspection required when all debris has been removed, before backfilling.
7. Within 5 working days of superstructure removal,a final inspection shall be requested. The
site shall be left clean and clear of all debris,with any excavation filled with earth level with
the adjacent ground elevation(except when such excavation is to be used as part of a new
building and such new building is actually under construction).
8. The undersigned owner shall and hereby does indemnify and hold harmless the City of
Orono, its agents, employees and assigns from and against all claims, damages, losses or
expenses,including attorney fees,against the City,its agents,employees and assigns arising
out of or resulting from the demolition described herein as performed by the property owner,
his employees, agents, subcontractors or assigns.
9. Septic systems must be abandoned per Minnesota Rules Chapter 7080. All septic tanks must
be pumped,crushed and filled with native soils. An inspection is required after the tanks are
pumped and before the tanks are crushed and filled.

To the property owner: Demolition of the principal dwelling structure on a
property may automatically terminate certain rights which may have accrued to the property by virtue of the
continued existence of that building.
>Rebuilding on a substandard lot of record(i.e. a lot that does not meet the zoning district required lot area
or width standards) will, with few exceptions, require variance approval by the City Council, and such
approval is not automatic nor guaranteed but requires that a hardship be demonstrated.
>Additionally, all current zoning standards will have to be met by the new principal dwelling including
setbacks, lot coverage by structures, hardcover(impervious surface), height limits, etc.
>Where municipal sewer is not available, provision of two (2) sites for a conforming on-site sewage
treatment system is mandatory.
>Unless specifically approved by the City,all accessory structures must be removed at the time of principal
dwelling demolition.

Dear Lyle,
I have compiled a summary of items and actions taken toward demolition at 1265
Bracketts Point Road:
1. Center Point Gas—disconnected at the Boulevard 9/4/03 —complete
2. Excel Energy—Electricity—disconnected 11/11/03 —complete
3. Well—Stadola pulled the pump and filled the well with concrete on 11/5/03 —
completed
4. Asbestos—Techtron Engineering—completed the removal on 9/19/03 —a copy of
report#F03-996 is attached
5. Metro Erosion Control—installed the erosion fencing (orange) on 11/12/03 —
complete
6. Sewer—Keith Clark at Westonka—will disconnect the sewer but due to an illness
has been unable to do so. It should be completed by 11/14/03.
A copy of the application and my check for$110.50 is attached. Thank you for your

Dear Mr. O'Shaugnessy:
Techtron Engineering was retained to remove asbestos containing materials from the house located at 1265
Bracketts Point Road, in Orono, MN. The project was completed on September 19, 2003.
Loca1 Area Protection
Danger signs were posted at access points to the house.
Removal of the pipe insulation in the basement and the garage was done using the "wrap and cut"
method. Full containments using negative air pressure machines was used far the removal of, 1) the
linoleum in the kitchen, back hall, and the back stairs, and 2)the paper in the interior walls.
Conclusions
The work was completed in accordance with applicable state and federal regulations.

INTRODUCTION
Techtron Ezigineering, Inc. was retained by Roger O'Shaugl�t�essy to pez£orm an asbestos survey
of the liome located at 1265 Bracketts Point Road in Orono, Minnesota.

performed, the infonmation in this report is to be used by ar� Asbestos Project Designer to
assist in the development of Project 5pecifications. Under no circurnstances is this repc�rt to
be used as a bid document.
Every material. iun the £acilzty was neither sampled nor analyzed. Rather, materials were
grouped homoger�eousay based on date of application, color, size, etc. Sam�les of each
homogeneous group were th�n taken and analy-r_cd for asbestos.
Standazds
The Occupatioaal Sa£ety and Health Administration (OSHA), the state of 1VPinnesota, and the
Env%ronmental Protection Agency (�PA) a1I have promulgated zegulations govenaing asbestos
in buildin.gs and/or asbestos exposur� in a working�nvironment.
OSH� standards state that personnel asbestos exposure must �,ot exceed O.l fi.bers per cubic
centimeter of air based on an S hour time wcight�d avErag�.
Minnesota regulations state that anv damaged, friable asbestos must be repaired and tha.t
asbestos must be evaluated prior to any planned zenowation. The Minnesota Department of
Health also has r.ules govez�ing asbestos abateznent. These rules gov�� worker training,
abaxement notifications, licensing,methodology, ete.
The E�A governs the disposa] of asbestos, air emissions, and notification procedures for
abatement. The EPA also govems asbestos in schools and asbestos ir�demolition pr.ojects.

Bullc samples were collected by wetting the material in question witb am�n�ded water. A,snnalJ.
portion of this material was then removed and placed directly in a sealed corrta.ine�r �ot
transport to Techtron Engineering's in-house taboratory_
Bulk Asbestos Analvsis
Analysis was performed by Teehtron Engizxeerizxg izi accordance to EPA methodology. T1�is
ztaethod detects asbestos in volumetric concentrations of 1% or greate�. Bulk samples were
checked fnr the following asbestifnrm minerals: ebrysotile, amosite, crocidolit�, anthophyllitc,
and tremoute-actinolite.
DISCUSSIpN
Bulk sampl� results and visual observations were used to identify and homogeneously group
materials within each area. Generalizations we7e then made on the types of znatez�ials that do or
do not contain asbestos.

The asbestos containin�g materials throughout the house must be removed befnre demolztioa -
There is no guarantee, iznplied or otherwise, that all asbestos containir�g materials have been
identified. If suspect materials are encounter�d durir�g zenovation and/or denlolition that have not
i�en addressed in this report, the material must not be disturbed until the suspect material zs
positively i,dentzfied.
OSHA 1926.1101 requises that all pezsonnel who may come into contact with asbestos contaiz�uzg
matezia]s must receive arinual 2-hour asbestos awazeness training.
