Sec.13.04 RIGHTS OF SUBTECTS OF DATA
Subd.l. Type of data. T6e rights of indi vidual on whom the data is stored or to be stored shall be as set forth in this sect ion.
Subd.2. Information required to be give n individual.An individual asked to supply private or confidential data concemiog h unself shall be
informed of: (a)the purpose and intended use of the requested data within the co llecting state agency,po]itical subdivision, or statewide system;(b)
whether he may refuse or is legally required to supply the re quested data;(c)any known conse quence arising from his supplying or refusing to supply
private or confidenrial data;and(d)the identiry of other persons or eorities authorized by state or federal law to receive t he data.This requirement s6a11
not apply when an individual is asked to supply investigative data,pursuant to sec tion 13.82,subdivision 5,to a law enforcem ent officer.

Subd.3. Access to data by i ndividual. Upon request to a res ponsible authority,an individual shall be informed whether he is the subject of

stored private or public data on individuals shal]be shown the data without any charge to him and,if he desires,shall be inf ormed of the content and
meaning of that data After an individua] has been shown the private data and informed of its meaning,the data need not be di sclosed to him for six
months thereafrer unless a dispute or action pursuant to this s ection is pending or additional da ta on the individual has been collected or created. The
responsible authority shal]provide copies of the private or pub ]ic data upon request by the indi vidual subject of the data. T he responsible authority
may require the requesting person to pay the actual w sts of making,certifying,and compiling the copies.
The respoc�sible au[hority shall comply immediately,if possible, with any request made pursuant to this subdivision,or withi❑ five days of

Subd.4. Procedure when data is not accurate or complete. An individual may contest the accuracy or completeness of public or private data
conceming himself. To exercise this ri eht,an individual shall notify in writing th e responsible authority describing the nat ure of the disa��reement.1'he
responsible authority shall within 30 days either. (a)correct the data found to be inaccurate or incomplete and attempt to no tify past recipients of
inaccurate or incomplete data,including r ecipients named by the individual;or(b) notify the iodividual that he believes the data to be conect. Data in
dispute shall be disclosed only if the individual's statement of disagreement is included with the disclosed data.
The determination of the responsible authoriry may be appealed pursuant to the provisions of the administrative procedure act r elating to
contested cases.

builder, but that was months ago. The escrow agreement is attached. I've removed your
husband's signature blank. You can mail it (PO Box 66 Crystal Bay 55323-0066), drop it
off (you can use the drop box after hours but let me know it's there), or give it to the builder
to bring when they come to pick up the permit.

Your project is subject to Orono regulations regarding Construction Site Runoff Control. The
purpose of these regulations is to protect wetlands and lakes. You are responsible for
complying with these regulations. The following is an abbreviated summary of the
regulations.
1. If your project requires a permit from the Minnehaha Creek Watershed District
(MCWD) or Minnesota Pollution Control Agency (MPCA), you must also apply for and
comply with those permit requirements.
2. Soil that accumulates behind a silt fence must be removed when it reaches a third of
the way to the top of the fence.
3. All disturbed ground left inactive more than 14 days must be either temporarily or
permanently stabilized (mulch, seed and mulch, sod, hydromulch, etc.)
4. You must provide sanitary facilities for all on-site workers, either a portable toilet or
access to the permanent facilities.
5. Refuse and materials to be recycled must either be taken off site at the end of each
work day or be placed in a dumpster. The dumpster must be emptied as necessary.
6. Vehicles must enter and leave the site by means of the existing driveway or the
stabilized construction entrance.
7. Construction parking must be located on site, on a paved roadway, or an improved
shoulder.
8. Any material tracked onto a roadway must be removed by the end of the work day.
Removed material must be returned to the project site or removed, not just swept off
the road.
9. Dumpsters and material may not be stored on a roadway.
