The Completed and signed "Application for Modification to Original Mortgage Document <br /> and/ or Collateral"form. <br /> 3. A survey, by a registered land surveyor, that clearly indicates the location of all <br /> improvements on the land and the area to be affected highlighted with an appropriate <br /> legal description. <br /> 4. A copy of the Easement Deed between the borrower and a third party. <br /> 5.
