Sec.13.04 RIGHTS OF SLTB.TECTS OF DATA
Subd. 1. Type of data. The rights of individual on whom the data is stored or to be stored shall be as set forth in this secdon.
Subd.2. Information required to be given individual. An individual asked to supply private or confidential data conceming himself shall
be informed of: (a) the purpose and intended use of the requested data within the collecang'state age�cy, polirical subdivision, or statewide system;
(b)whether he may refuse or is legally required to supply the requested data;(c)any known consequence arising from his supplying or refusing to supply
private or confidential data;and(d)the idendry of other persons or endries authori2ed by state or federal law to receive the data. This requirement shall
not apply when an individual is asked to supply investigarive data, pursuan[to seccion 13.82, subdivision 5, to a law enforcement o�cer.
The commissioner of revenue mav place the notice required under this subdivision in the individual income tac or propertv tax refund
instnicrions instead of on those forms.
Subd. 3. Access to data by individual. Upon request to a responsible authoriry,an individual shall be informed whether he is the subject
of stored data on individuals, and whether it is classified as public, private or confidential. Upon his fur[her request, an individual who is the subject
of stored private or public data on individuals shall be shown the data without any char;e[o him and, if he desires, shall be informed of the content
and meaning of that data. After an individual has been shown the private data and informed of its meaning, the data need not be disc(osed to him for
six months thereafter unless a dispute or acdon pursuant to this section is pending or addidonal data on the individual has been collected or created.
The responsible authoriry shall provide copies of the private or public data upon request by the individual subject of the data. The responsible authoriry
may require the requesting person to pay the actual cosu of making, cemfying, and compilin�the copies.
The responsible authority shall comply immediately, if possible, with any request made pursuant to this subdivision,or within five days of
the date of the request,excluding Saturdays,Sundays and legal holidays,if immediate compliance is not possible. If he cannot comply with the request
within that time,he shall so inform rhe individual,and may have an addidonal five days within which to comply with the request,excluding Saturdays,
Sundays and legal holidays.
Subd.4. Procedure when data is not accurate or complete. An individual may contest the accuracy or completeness of public or private
data conceming himself. To exercise this right,an individual shall noafy in writing the responsible authority describing the nature of the disagreemen[.
The responsible auchoriry shall within 30 days either: (a)correct the data found to be inaccurate or incomplete and attempt to nodfy past recipients of
inaccurate or incomplete data, including recipients named by the individual; or(b)nodfy the individual that he believes the data to be correct. Data
in dispute shall be disclosed only if the individual's statement of disagreement is included with the disclosed data.
The determination of[he responsible authoriry may be appealed pursuant to the provisions of the adminiscrative procedure act retating to
contested cases.

The purpose of this letter is to report on activities regarding the repair of this residence.

J. H. Dahlmeier Engineering Inc. has been retained to provide structural engineering and construction
review for the repairs followinb a fire for the resideiice at 3034 Casco Point Road, Orono, MN, as
directed by Al Pribyl of Giertsen Companies.
BACKGROUND
A fire destroyed much of the residence.
Giertsen Companies retained J. H. Dahlmeier Engineering Inc. to provide structural engineering for the
repair of the structure.
Construction documents are J. H. Dahlmeier Engineering Inc. drawings S-1 through S-5 dated 10-15-97.
CONSTRUCTION ISSUES
Firenlace
1. On a joint inspection with the City Building Officials, it was determined that the firepiace was not
constructed as previously thought and is non-conforming to Code. We mutually agreed to
remove the masonry to mezzanine level to determine the best solution.
2. It was determined that the fireplace masonry is basically an outside shell with numerous stacks
and flues. No separations exist between flues.
3. The mezzanine and first floor framing is directly attached to the fireplace masonry.

4. The City of Orono Building Inspectio��s Department is requiring construction consistent with
current Code since this is a major repair. The fireplace was not constructed in accordance with
the Code at the time of construction.
5. The following requirements were �uutually agreed upon:
a. Full masonry construction with partitions between tlues in accordance with the Code.
b. Support roof beam on steel column supported on solid masonry.
c. Framing for first floor and mezzanine on independent beam behind firepiace with 1" air
space.
d. First floor framing beams to be changed to steel can be supported on solid masonry of
fireplace due to its distance from the flue.
e. The barbecue will be abandoned.
Roof Frami���
6. Roof framing was changed with two 1-3/4" x 16" microlams spanning full length from south
exterior wall to north exterior wall and two interrnediate columns.
FINAL INSPECTION
7. A final site visit was conducted by John H. Dahlmeier of J. H. Dahlmeier Ensineering Inc. on
January 13, 1998. All construction is consistent with oribinal construction documents as
moditied and noted herein. All work is satisfactory.
8. The City of Orono building framing inspection has been accomplished with no discrepancies.

This total price will be held for a 30 day period. If an agreement has
not been reached with Giertsen Company to do the work by that date, then
Giertsen has the option to adjust the price for increases on labor,
material and project scope.
The price of this estimate is based on doing all the work listed. Changes
in the scope will be handled by written "Change Order. " The adjustment in
price will be based not only on the change, but also on how that change
will effect the other segments of the projects.

installed in accordance with the approved manufacturer's instructions.
310.9.1.2 Additions, alterations or repairs to Group R Occupancies. When the valuation of an
addition,alteration or repair to a Group R Occupancy exceeds $1,000 and a penmit is requixed, or
when oneor more sleeping rooms are added or created in existing Group R Occupancies, smoke
detectorsshall be installed in accordance with Sections 310.9.1.3, 310.9.1.4 and 310.9.1.5 of this
section.
EXCEPTION: Repairs to the exterior surfaces of a Group R Occupancy are exempt from the require-
ments of this section.
310.9.1.3 Power source. In new construction, required smolce detectors shall receive their pri-
mary power from the building wiring when such wiring is served frozn a commercial source and
shall be equipped with a battery bacicup. The detector shall emit a signal when the batteries are low.
Wiring shall be permanent and without a disconnecting switch other than those required for over-
current protection. Smolce detectors may be solely battery operated when installed in eacisting
buildings; or in buildings without commercial power; or in buildings which undergo alterations,
repairs or additions regulated by Section 310.9.1.2.
310.9.1.4 Location within dwelling units. In dwelling units, a detector shall be installed in each
sleeping room and at a point centrally located in the corridor or area giving access to each separate
sleeping area. When the dwelling uxut has more than one story and in dwellings with basements, a
detector shall be installed on each story and in the basement. In dwelling units where a story or base-
ment is split into two or more levels, the smolce detector shall be installed on the upper level, except
that when the lower level contains a sleeping area, a detector shall be installed on each level. When
sleeping rooms are on an upper level, the detector shall be placed at the ceiling of the upper level in
close proxi�mity to the stairway. In dwelling units where the ceiling height of a room open to the
haIlway serving the bedrooms exceeds that of the hallway by 24 inches (610 mm) or more, smoke
detectors shall be installed in the hallway and in the adjacent room. Detectors shall sound an alarm
audible in all sleeping areas of the dwelling unit in which they are located.

Handrails are required on interior and exterior stairs with
4 or more risers.
Handrails shall be placed not less than 34 inches nor more
than 38 inches above the nosing of treads. They shall be
continuous the full length of the stairs. Ends shall be returned
or shall terminate in newel posts or saftey terminals.
The handgrip portion of handrails shall be not less than 1
1/2 inches or more than 2 inches in cross-sectional dimension or
the shape shall provide an equivalent gripping surface. The
handgrip portion of handrails shall have a smooth surface with no
sharp corners.
Handrails projecting from a wall shall have a space of nvt
less than 1 1/2 inches between the wall and the handrail.
