Sec.13.04 RIGHTS OF SUBJECTS OF DATA
Subd.1. Type of data. The rights of individual on whom the data is stored or to be stored shall be as set forth in this section.
Subd.2. Information required to be given individual.An individual asked to supply private or coofidential data concerniog himselfshall be
informed of: (a)the purpose and intended use of the requested data within the collecting state agency,political subdivision,or statewide system;(b)
whether he may refuse or is legally required to supply the requested data;(c)any known consequence arising from his supplying or refusing to supply
private or con5deotial date;and(d)the identity of other persons or entities authorized by state or federal Iaw to receive the data.This requirement shall
not appiy when an individual is asked to suppiy investigative data,pursuant to section 13.82,subdivision 5,to a law enforcement officer.

instructions instead of on those forms.
Subd.3. Access to data by individuaL Upon request to a responsible authority,an individual shall be informed whether 6e is the subject of
stored data oo individuals,and whether it is classified as public,private or confidential. Upoo his further request,ao individual who is the subject o[
stored private or public data on individuals shall be s6own the data without any c6arge to him and,if he desires,shall be informed otthe content and
meaning of that data. After an individual has been shown the private data and informed of its meaning,the data need not be disclosed to him for six
months thereafter unless a dispute or action pursuant to this section is pending or additional data on the individual has been collected or created. 7'he
responsibie authority shall provide copies of the private or public data upon request by the individual subject ofthe data.T6e responsibk authority may
require the requesting person to pay the actual costs of making,certifying,and compiling the copies.
The responsible authority shall comply immediately,if possible,with any request made pursuant to this subdivision,or within 5ve days of the

Subd.4. Procedure when data is not accurate or complete.An individual may cootest the accuracy or completeness of public or private dats
concerning himself.To exercise this right,ao individual shall notify in writing the responsible authority describing the nature of the disagreement The
responsible authority shall within 30 days either: (a)wrrect the data found to be inaccurate or incomplete and attempt to ootify past recipients of
inaccurste or incomplete data,including recipients named by the individual;or(b)ootify t6e individual that he believes the data to be correct Deta in
dispute shall be disclosetl ooly if the individual's stetemeot of disagreement is inciuded with the disclosed data.
The determination of the responsible authority msy be appealed pursuant to the provisions of the administrative procedure aM relating to
contested cases.

The purpose of this letter is to report the findings of a structural engineering review of the
frvst protection at the c4nstruction for the breezeway.
� ASSIGWMENT
Ulteig Engineers Inc. (UEI� has been retained to provide a structural engineering review of
the frost protection for the wall cvnstruction at the breezeway for the residence located at
3165 Casco Circle, Orono, MN, as directed by Scott Stevensan. the homeowner.

The owner recently enclased the breezeway area between the home and the garage with

�'he City Building Ins�ections Department wishes to have an independent structur�l
engineering review of the frost protectfon under the new walls constnicted.
OBSE�tVATIONS AND COMMENTS
1. The following in��rmation was obtained through a site insp�ction an March 16, 2004
by Jas�n Hanlon. of UEI.
a. The breezeway area is approximately 8'-6"wide and 12'-0° fong.
b. The clear r,eiling height is a�pproximately e'-0"
c_ The fallowing applies to the construction of the encfosed breezeway
according to the homeowner:

i. The roaf that is currently in ptace is the original roof that was there
prior to enclosing the walls.
ii. The breezeway is endosed with 2x wood framed wa11s that are tied to
the roof and ariached to the original concrete slab-4n-grade.
iii. The walls have 2x treated sill plates anchored to the slab-on-grade.
iv. There is no foundation wall below the exteriorwalls of the breezeway.
v. The new enclos�d area wi(I serve as a mud room and will be heated.
ENGINEERlNG OPINION
2. The exterior walls of the breezeway are exposed to potentlal frost heave since there
are no foundation walls below them to provide frost protection. The State Building
Code r�quires that adequate frost protection be provided on the exterior side of an
exterior wall for he�ted spaces. If the walls are subjected to any significant frost
heave, it may damage the roof and any corresponding connections to the main
framing of the house. Corrective measures are required to provide adequate frost
prvtection.
3. The Canadian Research Council and ASGE have determined through st�dies that
rigid insulation laid flat provides protection t4 footings against frost action. They
recommend installing rigid insulation. 4" thick, extending 4 feet horizontafly away
from any unprotected elements.
CORRECTIVE MEASURES
4. The following c:orrective measures are required:
a. 4-inches of rigid Styrofoam insulatian shalf be placed horizontalfy, so it �
e�ends 4 feet beyond the base vf the exteriorwall.
b. The existing exterio� concrete sl�b-on-grade shal( be �emoved and the
insulation shall be placed at a depth no greater then 6-inches. A new slab-
on-grade can be placed aver the rigid insulation.
PROFESSIONAL OPINION
5. It is my professio�al engineering opinion that;
a. The rigid insulation wil) provide adequate frost protection for the walls of the
breezeway.
GENERAL
6. The infarmation, observations, and opinions stated in this repo�t are based on an
inspection made by Jason Hanlon. The inspection consisted of a visual walk-
threugh observing expvsed elements and those accessible without the removal of
finished materials.
