Sec13.04 RIGHTS OF SUBJECTS OF D:1TA
Subd. 1. Type of data. The rights of individual on whom the data is stored or to be stored shall be as set forth in this section.
Subd.2. Infonnation required to be given individual. An individual asked to supply private orconfidential data concerning himselfshall be
infonned of. (a)the purpose and intended use of the requested data within tlie collecting state agency,political subdivision,or statewide system;(b)
whether he may refuse or is legally required to supply lhe requested data;(c)any known consequence arising from his supplying or refusin�to suppiy
private or confidential data;and(d)the identity ot other persons or entities authorized by state or federal faw to receive the data. This requirement shall
not apply wlten an individual is asked to supply investioa[ive data,pursuant to section 13.32,subdivision i,to a la�a enforcement ofticer.
The commissioner of revenue mav place the notice required under this subdivision in the individual income tax or propertv tas refund
instructions instead of on those forms.
Subd.3. Access to data by indi��iduaL Upon request to a responsible autliority,an individual shall be infonned whether he is the subject of
stored data on individuals,and�mhed�er it is classit5ed as public,private or contidential. Upon his fuRher request,an individual who is the subject of
stored private or public data on individuals shall be shown the data without any charge to him and,if he desires,shall be informed ofthe content and
meaning of that data. After an individual has been shown the private data and infonned of its meaning,the data need not be disclosed to him for six
months thereafter unless a dispute or action pursuant to this section is pending or additional data on the individual has been collected or created. The
responsible authority shall provide copies ofthe private or public data upon request by the individual subject ofthe data. The responsible authority
may require the requesting person to pay the actual costs of making,certifying,and compiling the copies.
The responsible authority sliall compfy immediately,if possible,�mith any request made pursuant to this subdivision,or�vithin tive days of
the date of the request,e�cluding Saturdays,Swtdays attd legal holidays,ifimmediate compliance is not possible, Ifhe cannot comply with the request
within that time,he shall so infonn the individual,and tnay have an additional tive days�i�ithin which to comply with the request,escluding Saturdays,
Sundays and legal holidays.
Subd.4. Procedure when data is not accurate or complete. An individual may contest the accuracy or completeness ol'public or privatedata
conceming himself. To exercise this right,an individual shal I notity in writing the responsible authoriry describing the nature of the disagreement. The
responsible authority shali within 30 days eithec (a)correct the data found to Ue inaccurate or incomplete and attempt to notify past recipients of
inaccurate or incomplete data,induding recipients named by the individual;or(b)notify the individual diat he believes the data to be correct. Dafa in
dispute shall be disclosed only if the individual's statement of disa�reement is included with the disclosed data.
The determination of the responsible authority may be appealed pursuant to the provisions ofthe administrative procedure act relating to
contested cases.

We have reviewed the revised survey for the�roposed house at 3799 Casco Avenue, dated 2-13-07. We
have the following comments with regards to engineering matters:
• The plan shows the proposed 947 contour to the southeast of the house crossing an existing 948
contour. The grading should be revised in this area such that no contour lines cross.
• The attached detail (STO-4) should be used for the proposed catch basin/grate.
• The proposed inlet should be protected with silt fence until vegetation is established.
