Sec.13.04 RIGHTS OF SUBJECTS OF DATA
Subd. 1. Type of data. The righ[s of individual on whom the data is stored or to be stored shall be as set forth in this section.
Subd.2. information required to be given individual. An individual asked to supply private or confidential dataconcerning himselfshall be
informed of: (a)the purpose and intended use ofthe requested data within the collecting state agency,political subdivision,or statewide system;(b)
whether he may refuse or is legally required to supply the requested data;(c)any known conseq�ence arising from his supplying or refusing to supply
private or confidential data;and(d)the identity of other persons or entities authorized by state or federal law to receive the data.This requirement shall
not apply when an individual is asked to supply investigative data,pursuant to section 13.82,subdivision 5,to a law enforcement officer.
The commissioner of revenue mav nlace the notice required under this subdivision in the individual income tax or property tax refund
instructions instead of on those forms.
Subd.3. Access to data by individual. Upon request to a responsible authority,an individual shall be informed whether he is the subject of
stored data on individuals,and whether it is classified as public,private or confidentiaL Upon his fuRher request,an individual who is the subject of
stored private or public data on individuals shall be shown the data without any charge to him and,if he desires,shall be informed of the content and
meaning of that data. After an individual has been shown the private data and informed of its meaning the data need not be disclosed to him for six
months thereafter unless a dispute or action pursuant to this section is pending or additional data on the individual has been collected or created. The
responsible authority shall provide copies of the private or public data upon request by the individual subject of the data. The responsible authority
may require the requesting person to pay the actual costs of making,certifying,and compiling the copies.
The responsible authority shall comply immediately,if possible,with any request made pursuant to this subdivision,or within five days of
the date ofthe request,excluding Saturdays,Sundays and legal holidays,if immediate compliance is not possible. If he cannot comply with the request
within that time,he shall so inform the individual,and may have an additional five days within which to comply with the request,excluding Saturdays,
Sundays and legal holidays.
Subd.4. Procedure when da[a is not accurate or complete. An individual may contest the accuracy or completeness of public or private data
conceming himself. To exercise this right,an individual shall notify in writing the responsible authority describing the nature ofthe disagreement. The
responsible authority shall within 30 days either (a)correct the data found to be inaccurate or incomplete and attempt to notify past recipients of
inaccurate or incomplete data,including recipients named by the individual;or(b)notify the individual that he believes the data to be correct. Data in
dispute shall be disclosed only if the individual's statement of disagreement is included with the disclosed data.
The detennination of the responsible authority may be appealed pursuant to the provisions ofthe administrative procedure act rela[ing to
contested cases.

SCOPE OF WORK
THESE YEAR 2007 THERMOMASS INSULATED POURED-IN-PLACE (PIP) CONCRETE
FOUNDATION DRAWINGS, CONSISTING OF S-1 THROUGH S-8 AND DATED 2-12-07, ARE
PREPARED BY ULTEIG ENGINEERS, INC. THESE DRAWINGS CONTAIN STRUCTURAL DESIGN
INFORMATION AND DETAILS TO BE USED IN THE CONSTRUCTION OF THERMOMASS
INSULATED POURED-IN-PLACE CONCRETE FOUNDATIONS FOR VARIOUS CONDITIONS
COMMON WITHIN THE RESIDENTIAL MARKET ONLY. THE INFORMATION IS INTENDED FOR
THE CONCRETE CONTRACTOR, AS WELL AS THE GENERAL CONTRACTOR AND THE

1. ALL SILL PLATES ARE TO BE ATTACHED TO THE WALLS WITH A MINIMUM OF TWO ANCHORS PER
SILL SECTION. ALL SECTIONS ARE TO HAVE AN ANCHOR WITHIN 4" TO 12" OF EACH END. AN
ANCHOR IS TO BE PI.ACED AT ALL WALL CORNERS AND INTERSECTIONS ON WALLS MORE THAN
TWO FEET IN LENGTH. WALLS TWO FEET OR LESS IN LENGTH REQUIRE A MINIMUM OF ONE ANCHOR.
SHEAR WALLS BY OTHERS MAY REQUIRE ADDITIONAL ANCHORAGE.
2. SILL PLATES ARE NOT REQUIRED TO BE TREATED IN ACCORDANCE WITH IRC SECTION 323. ULTEIG
RECOMMENDS INSTALLING A PROTECTION BARRIER BETWEEN THE SILL AND THE CONCRETE.
3. FLOOR MEMBERS ARE TO BEAR A MINIMUM OF 3.5" ON THE SILL PLATE. INSTALL CONTINUOUS RIM
BOARD, BLOCKING, AND CONNECTORS AS SHOWN IN DETAILS S3 AND S4. ANCHOR BOLTS MUST BE
A MINIMUM OF 2.5" FROM EACH EDGE OF THE SILL PLATE. INCREASE THE SILL PLATE SIZE AS
REQUIRED TO MEET THESE LIMITS AND THE REQUIREMENTS OF THE ENERGY CODE OR THE NEEDS
OF A PARTICULAR PROJECT.
4. ANCHOR BOLTS 0.5" OR LARGER IN DIAMETER DO NOT NEED TO BE GAIVANIZED OR OTHERWISE
PROTECTED FROM CORROSION IN ACCORDANCE WITH IRC SECTION R319.3. AL� FASTENERS AND
PREMANUFACTURED METAL CONNECTORS IN CONTACT WITH TREATED LUMBER ARE TO MEET ALL

COLD WEATHER CONCRETING GUIDELINES
1. THE CONTRACTOR IS TO WORK WITH THEIR CONCRETE SUPPLIER TO OBTAIN A MIX WHICH WILL
ADEQUATELY PERFORM UNDER THE EXPECTED CONSTRUCTION CONDITIONS. THE USE OF
ADDITIONAL CEMENT, HIGH-EARLY STRENGTH CONCRETE, AND/OR ACCELORATOR IS
RECOMMENDED AT TEMPERATURES LESS THAN TWENTY DEGREES F.
2. DELIVER THE CONCRETE TO THE SITE WARM AND IN A TIMELY MANNER. PLACE CONCRETE EARLIER
IN THE DAY, IF POSSIBLE.
3. DO NOT ADD EXCESSIVE WATER TO THE CONCRETE. THE USE OF WATER-REDUCING AGENTS IS
RECOMMENDED WHEN WORKABILITY IS OF CONCERN.
4. WALL BLANKETS, OR OTHER ACCEPTIBLE MEANS OF PROTECTION, ARE HIGHLY RECOMMENDED,
OVER THE TOP 24" OF THE WALL, WHEN THE AIR TEMPERATURE IS LESS THAN ZERO DEGREES F.
5. NO CONCRETE IS TO BE PLACED ON SNOW OR IN CONTACT WITH FR02EN MATERIALS.
6. REBAR DOES NOT NEED TO BE PRE-HEATED, BUT IS TO BE FREE OF FROST, SNOW, AND ICE AT THE
TIME OF CONCRETE PLACEMENT.
