Sec.13.04 RIGHTS OF SLJBJECTS OF DATA �.J '
Subd. I. Type of data. The rights of individual on whom the data is stored or to be stored shall be as set focth in this section. + '
Subd.2. Information required to be given individual. An individual asked to supply private or confidential data conceming himself shall
be infonned of: (a)the purpose and intended use of the requested data within the collecring state agency, policical subdivision, or statewide system; (b)
whether he may refitse or is legally required to supply ihe requested data; (c)any known consequence arising from his supplying or refusing to supply
private or confidenrial data; and (d)the idendry of other persons or enrities authorized by state or federal law to receive the data. This requirement shall
not apply when an individual is asked to supply investigative data, pursuant to section 13.82, subdivision 5, to a law enforcement officer.
The commissioner of ievenue mav place tf�e notice iequired under dils subdivision in the individual income tax or oropertv tax refund instrucrions
instead of on those forms.
Subd.3. Access to data by individual. Upon request to a responsible authoriry, an individual shall be informed whether he is the subject of
stored data on individuals,and whether it is classified as public,private or confidential. Upon his further request, an individual who is the subject of stored
private or public data on individuals shall be shown the data without any charge,to him and, if he desires, shall be informed of the content and meaning
of that data. After an individual has been shown the private data and informed of its meaning, the data need not be disclosed to him for six months
theceafter unless a dispute or acaon pursuant to this section is pending or addiaonal data on the individual has been collected or created. The responsible
au�ority shall provide copies of the private or public data upon request by the individual subject of the data. The responsible authority may require the
requesang person to pay the actual costs of making, certifying, and compiling the copies.
The responsible authoriry shall comply immediately, if possible, with any request made pursuant to this subdivision, or within five days of the
date of the request,excluding Satunjays,Sundays and legal holidays, if immediate compliance is not possible. If he cannot comply with the request within
that time,he shall so infonn the individual, and may have an addiuonal five days within which to comply with the request, excluding Saturdays, Sundays
and legal holidays.
Subd.4. Procedure when data is not accurate or complete. An individual may contest the accutacy or completeness of public or private data
conceming himself. To exercise this right, an individual shall nodfy in writing the responsible authoriry describing the nature of the disagreement. The
responsible authority shall within 30 days either: (a)correct the data found to be inaccurate or incomplete and attempt to notify past recipienu of inaccurate
or incomplete dara,including recipienu named by the individual; or(b)notify the individual that he believes the data to be correct. Data in dispute shall
be disclosed only if the individual's statement of disagreement is included with the disclosed data.
The determinadon of the responsible authoriry may be appealed pursuant to the provisions of the administrative procedure act relating to contested
cases.

planning; however, City Council approved connection to City sanitary sewer on 01/10/00;
and it is our intention to connect to City sewer.

5. Page 2, Item 5: Lyman Lumber and Automated Building Components (ABC) are in the
process of generating roof truss layout and designs. That information will be forwarded to
you when completed.
6. Page 2, Item 5: Refer to the Architectural Drawings for window and door schedules. As
noted in Item 4 above, Marvin Windows are scheduled, but the actual window and door
manufacturer will be Windsor Windows. Actual unit and rough opening dimensions may
vary up to one inch (1"+/-).
7. Page 2, Item 5: The one (1) skylight has been deleted from the project.
8. Page 2, Item 5: Stairways will be custom-fabricated on site.

15. Page 5, Item 3: Offset stakes will be located when the building is staked on the site. This
office has directed the Surveyor to stake the house and the Surveyor stated that such work
will be done in the next couple of days.
16. Page 5, Item 8: Elevation at top of curb along North Brown Road has not been indicated.
It is the Surveyor's assumption that such a requirement is typically for much smaller parcels.
If curb elevation is required, please advise this office, and work will be performed
immediately.

floor elevations.
18. Page 5, Item 11 : Elevation at each property corner has not been indicated. Again, it is the
Surveyor's assumption that such a requirement is typically for much smaller parcels.
Additionally, we will not be grading near property corners, except at driveway near
southeast corner. If corner elevations are required, please advise this office, and work will
be performed immediately.
19. Page 5, Item 12: Existing structures on adjacent parcels have not been indicated. The
Surveyor is unsure if such information is required for this lake parcel. If locations of existing
adjacent structures is required, please advise this office, and work will be performed
immediately. Such drawing will probably be required to be at a much smaller scale,
because some adjacent structures are quite distant from this parcel.
20. Page 5, Item 15: Established high water elevation of Dickey Lake is not shown on the Land
