treatr>ant system at the above addreu has been inspected and appears to fail into the category checked below.

Tank inspection indicates;
□ Pumpout not needed at this time.
□ Solids accumulation in tanks indicates they should be pump
ed out this year to help prevent future problems.
□ Solids accumulation in tanks is at a critical level. Tanks
should ba pumped out at toon at pouibla.
□ System it ditchargir>g to surface. Tanks mutt ba pumped
within 48 hours to eliminate surface discharge.
□ Inspection risers missing-tanks could not ba inspected.
Inspection risers (4** dia. pipe) mutt be installed in each tank
at itext pumpout. If tanks have not been pumped out within
tfie last three years, they should be pumped out now.

have not been pumped out within the last three years, they
should be pumped out now.
Drainfield inspection indicates;
Drainfield it dry, no surfacing evident.
□ Some evidence of surfacing, not critical yat.
□ Drainfield it saturated and visibly discharging untreated
effluent to the surface. This condition may require replace-
rrrent or additions to drainfield. Contact the City Inspector
immediately. Repairs mutt ba completed within 90 days.
