' THIS iS NOT A PERMIT. This is a design approval form which must accompany the site plan. A pennil trust
be issued to a licensed septic contractor prior to installation. A list of currently licensed septic contractors Is
enclosed.
NOTICE TO iNS'i ALLE—F Any ;hanger, to the approved plans and specs must havo prior approval of the
Inspector (473-7357). Call for inspections 1.4 hours itt ldvance.
ALL DRAINFIfsLD AI;rAS MUST 131 FENCED OFF prior to building site excavation and fencing must remain
in place until finai site grading. Approval to pour footings will not be granted until the Inspections Department
has verified that primary and alternate sites are adequately protected.
NO VUJICULAR TRAFFIC: OF ANY KIND (cars, trucks, earth moving equipment, etc.) is allowed within 20'
of tested drainfield sites either before or after system construction. Compaction of these areas could render them
unusable prohibiting the titnely completion and or lintitiug the long term use of the property.
A site copy will be available at the City Offices for the septic rontractor.
