bed if the site is located south and west of the proposed site. Either
site may be used as an alternate.
THIS IS NOT A PERMIT. This is a design approval form which must accompany the site plan. A permit must
be issued to a licensed septic contractor prior to installation. A list of currently licensed septic contractors is
ci.closed.
NOTICE TO INSTALLERS: Any changes to the approved plans ,.nd specs inust have prior approval of tote
Inspector (473-7357). Call for inspections 24 hours in advance.
ALL DRAW FIELD AREAS MUST BE FENCED OFF prior to building site excavation and fencing must retrain
in piace until final site grading. Approval to pour footiul,s will not be granted until the Inspections Depurtinent
has verified that primary and alternate sites are adequately protected.
NO VEHICULARTRAFFIC OF ANY KIND (cars, trucks, earth nioviug equipment, etc.) is allowed within 20'
of tested drainfield sites either before or after system construction. Compaction of these areas could render them
unusable prohibiting the timely cuniplction and or limiting the long term use of the property.
