creating a safety haaard, or is otherwise creating a public nuisance. Please contact the City Inspector to discuu system repair/
replacement procedures. If drainlield replacement it necetsaiy, toil testing will usually be required, and a design and site plan
must be submitted for review. Your contractor mutt obtain a permit before work it started.

Tank inspection indicetet:
- '0 Pumpout not needed at this tinte.
□ Solids accumulatian in tanks indicates they should be pump*
ad out this year to help prevent future proMems.
□ Solids accumulation in tanks is at a critical level. Tanks
should be pumped out at toon at possible.
□ System it discharging to surface. Tanks mutt be pumped
within 48 hours to eliminate surface discharge.
□ Inspection risers missing-tanks could not be inspected.
Inspection risers (4" die. pipe) mutt be installed in each tank
at next pumpout. If iei«ks have not been pumped out within
the last three years, they should be pumped out now.
□ Inspection pipe it located directly over tank baffle. (Does not
give accurate measurement of solids accumulation.) If tanks
have not been pumpad out within the last three years, they
should be pumpad out now.
Drainfiald inspection indicatet:
-----B Drainfiald is dry, no surfacing avidant.
□ Some evidence of surfacing, not critical yet.
□ Drainfiald is saturated and visiblv discharging untreated
affluent to the surface. This condition may require replace*
ment or additions to drainfiald. Contact the City Inspector
immediately. Repairs mutt be completed within 90 days.
□ Drainfiald extent and condition unknown.
