City Ordinance No. 210 requires that each on -site sewage trestment system in Orcno be inspected on a regular basis. The on -site sewage
treatment system at the abcioee vaddress has been inspected and appears to fall into the category checked below.

located, and constructed generally in accordence with previous codes. System appears to be functioning property; no major
upgrading of the system is required at this time,

adequately at I-os time, but has a relatively high potential for future problems. No major upgrading of system is require, at
