This is a design approval form which must accompany the site plan. A permit must <br />be issued to a licensed septic contractor prior to installation. A list of currently licensed s^c contractors is <br />enclosed. <br />NOTICE TO INSTALLERS: Any changes to the approved plans and specs must have prior approval of the <br />Inspector (473-73S7). Call for inspections 24 houra in advance. <br />ALL DRAINFIELD AREAS MUST BE FENCED OFF prior to building site excavation and fencing must reiuain <br />in place until final site grading. Approval to pour footings will not be granted until the Inspections Department <br />has verified that primary and alternate sites are adequately protected. <br />NO VEHICULAR TRAFFIC OF ANY KIND (cars, trucks, earth moving equipment, etc.) is allowv i within 20* <br />of tested drainfield sites either before or after system cofL:truction.
