treatment system at the above addreu hat been inspected and appears to fall into the category checked below.

properly.

replacement procedures. If drainfield replacement is necessary, toil tettirtg will usually be required, and a design and site plan
mutt be submitted for review. Your contractor mutt obtain a permit before work it started.

□ Solids accumulation in tanks indicates they should be f
ed out this year to help prevent future problems.
□ Solids accumulation in tanks is at a critical level. Tanks
should be pumped out at toon at pouible.
□ System it discharging to surface. Tanks mutt be pumped
within 48 hours to eliminate surface discharge.
□ Inspection risers miuing-tankt could not be inspected.
Inspection risers (4" die. pipe) mutt be installed in each tank
at next pumpout. If tanks have not been pumped out within
the last three years, they should be pumped out now.
□ Inspection pipe it located directly over tank baffle. (Does not
give accurate measurement of solids accumulation.) If tanks
have not been pumped out within the laet three years, they
should be pumped out now.
Drained inspection indicates
Orainfield is dry, no surfacing evident.
□ Some evidence of surfacing, not crittral yet.
G Drainfield is saturated and visibly dischargirtg untreated
effluent to the surface. Thi* cnnrfition may require replace
ment or additions to drainfield. Contact the City Inspector
immediately. Repairs mutt be completed within 90 days.
