THIS IS NOTA PERMIT. This is a design approval form which must accompany the site plan. A permit must
be issued to a licensed septic contractor prior to installation. A list of currently licensed septic contractors is
enclosed.
NO'1':Cf? '1'0 INS'I'AI.1.I;ItS: Any changes to the approved plans and specs must have prior approval of the
Inspector (473-7357). Call for inspections 24 hours in advance.
ALL DRAINFIELD A11F.AS MUSTBE FENCED OFF prior to building site excavation and fencing must remain
in place until final site grading. Approval to pour footings will not be granted until the Inspections Department
has verified that primary and altcnnate sites are adequately protected.
NO VEIIICU LAIt TRAFFIC Or ANY KIND (cars, trucks, earth moving equipment, etc.) is allowed within 20'
of tested drainfield sites tither before or after system construction. Compaction of these areas could render them
unusable prohibiting the timely colnplclion and of limiting the long term use of the property.
A site copy will In- available at the City Offices for the septic contractor.
