Drainfield is saturated and visibly discharging untreated effluent to the surface. Contact
the City Inspector immediately. Repair must be completed within 90 days.

COMMENTS: The septic system is non-compliant, but does not appear to pose an imminent threat to
public health or safety (surface discharge or backup into the dwelling). The system must be
repaired or replaced by 12/31/2005 or within ninety days of posing an iraminent threat to public
health or safety, whichever comes first.
