A Compliance inspection was completed for the existing on-site sewage treatment
system located on this property. The system consists of 1 - 750 gallon trash trap, 1 -
1000 gallon dose tank, 1- 750 gallon Multi -Flo Pre -Treatment unit, 1 -1500 gallon
pump tank that pumps to the Type 111 pressurized mound system with a 12' x 50'
rock bed (only get credit for 10' x 501) built an 6-29-2006, sized for a 4 bedroom home.
The mound was placed on fill soil with redox features found in the fill soil, elev. 99.7
& the bottom of the rock at elev. 100.8 leaving a 1.1' separation from the bottom of
the rock & redox features. This system is classified as non-compliant due to not
meeting a 3' separation required. The blue -gray colors found indicate sewage
ponding in the fill. The sand in the mound extends 11' out from the rock bed leaving
a 32' wide absorption area (should be 501).
The original design completed by SP Testing, Inc. was to remove the fill soil & install
a 1500sq.ft, seepage bed for a 5 bedroom home, removing the fill soil & replacing it
with clean washed sand.
This design was changed by Willy Gibbs, Inspector for the City of Orono at the time.
At the time of the installation. Willy did not like the design & changed it. No new
design has been found. I was very concerned about the change of the design & it
was decided that Willy would do the monitoring of the system. No monitoring
information was found at the City of Orono.
The tanks were pumped by Elmer J. Peterson Co. & were found to be compliant.
The Multi -Flo has been serviced by Schirmers Wastewater Treatment Systems since
it was installed. The Multi -Flo Unit requires to be serviced 2 times a year at a cost of
$275.00 a year (2024 price).
To replace the system, a full site evaluation & Performance Design will need to be
completed prior to installation.
The shrubs around the tank must be removed to prevent the roots from causing
damage to the tanks & also for servicing the unit.

Purpose: This form may be used to certify the compliance status of the sewage tank components of the SSTS. This form is not a
complete SSTS inspection report, only a tank integrity assessment, and may only certify sewage tank compliance status
when entirely completed and signed by a qualified professional. SSTS compliance inspection report forms can be found at:

Instructions; This form may be completed, and signed, by a Designated Certified Individual (DCI) of a licensed SSTS inspection,
maintenance, installation, or service provider business who personally conducts the necessary procedures to assess the compliance
status of each sewage tank in the system. Only a licensed maintenance business is authorized to pump the tank for assessment. A
copy of this information should be submitted to the system owner and be maintained by the licensed SSTS business for a period of
five (5) years from the assessment date.
When this form is signed by a qualified certified professional, it becomes necessary supporting documentation to an Existing

The information and certified statement on this form is required when existing septic tank compliance status is determined by an
individual other than the SSTS Inspector that submits an inspection report. This form represents a third party assessment of SSTS

beyond the signature date on this form unless a new evaluation is requested by the owner or owner's agent or is required according
