Sec.13.04 RIGHTS OF SUB.JECTS OF DATA
Subd. 1. Type of data. The rights of individual on whom the data is stored or to be stored shall be as set forth in this section.
Subd.2. Information required to be given individual. An individual asked to supply private or confidential data concerning himself
shall be informed of: (a)the purpose and intended use of the requested data within the collecting state agency,political subdivision,or statewide
system;(b)whether he may refuse or is legally required to supply the requested data; (c)any known consequence arising from his supplying or
refusing to supply private or confidential data;and(d)the idenrity of other persor�s or entiUes authorized by stare or federal law ro receive the data.
This requirement shall not apply when an individual is asked to supply investigative data, pursuant to section 13.82, subdivision 5, to a law
enforcement officer.
The commissioner of revenue mav vlace the notice reauired under this subdivision in the individual income tax or oropertv tax refund
instructions instead of on those forms.
Subd. 3. Access to data by individual. Upon request to a responsible authoriry, an individual shall be informed whether he is the
subject of stored data on individuals, and whether it is classified as public, private or confidential. Upon his further request, an individual who
is thz subject of stored private or pubfic data on individuals shall be shown the data without any charge to him and, if he desires, shall be informed
of the content and meaning of that data. Afrer an individual has been shown the private data and informed of its meaning, the data need not be
disclosed ro him for six months thereafter unless a dispute or action pursuant to this section is pending or additional data on the individual has been
collecred or created. The responsible authoriry shall provide copies of the private or public data upon request by the individual subject of the data.
The responsible authoriry may require the requesting person ro pay the actual costs of making,certifying,and compiline the copies.
The respocuible authoriry shall comply immediately,if possibie,with any request made pursuant to this subdivision, or within five days
of the date of[he request, excluding Saturdays,Sundays and legal holidays, if immediate compliance is not possible. If he cannot comply with
the request within that time, he shall so inform the individual, and may have an additional five days within which to comply with the request,
excluding Samrdays, Sundays and legal holidays.
Subd.4. Procedure when data is not accurate or complete. An individual may contest the accuracy or completeness of public or
private data concerning himself. To exercise this right, an individual shall notify in writing the responsible authoriry describing the nature of the
disagreement. The responsible authoriry shall within 30 days either. (a)correct the data found to be inaccurate or incomplete and attempt to notify
past recipients of inaccurate or incomplete data, including recipients named by the individual;or(b)notify the individual that he believes the data
to be correct. Data in dispute shall be disclosed only if the individual's statement of disagreement is included with the disclosed data.
The determina[ion of the responsible authoriry may be appealed pursuant to the provisions of the administra[ive procedure act relating
to contested cases.

Demolition
• Remove all gray oak cabineuy in kitchen, family room,and buder��. Remove tall gray oak
cabinet on west wall of laundry room. Place all intact cabinetry in detached garage for pickup.
• Remove existing light fixtures and ceiling fan in kitchen and budery.
• Remove existing appliances from current location. Destinations are as follows:
♦ Detached garage for pickup- kitchen sink,bar sink,refrigerator in budery,cooktop
♦ Attached garage for permanent use- refrigerator in kitchen
♦ Temporary spot until reinstalled-double ovens, dish washer, trash compactor,disposal,
and microwave oven
♦ Remove and dispose of carpet in family room and wood floor in kitchen and budery.
• Remove all door casings,window casings,and baseboards in kitchen,but]ery and family room.
Remove crown moulding in budery where it will interfere with new cabinetry.
♦ Remove all wallpaper in kitchen and butlery.
♦ Properly dispose of and remove all scraps and trash associated with demolition and new
construction.
Patio Door
♦ Remove windows (approx 8.5 feet wide) above counter on east wall of former kitchen and
create space for patio door.
♦ Supply and install Cresdine CrestClad Hinged Patio Door I.FW4-2366L0 (Rough opening 8'
0.81'� or Cresdine CrestClad Hinged Patio Door LFW4-2866L0 (Rough opening 8' 10.81'�.
� Specify plan for filling or expanding opening to accommodate door width. Specifically, include
details of whether you will need to 1)replace existing header to expand opening,2)fill in exterior

• Note --We assume door height is 6'8" but door is available in 6'l l" if opening is higher. Also,

♦ Supply and install sub-floor suitable for stone tiling throughout family room, kitchen,and
budery. Provide details on what eYacdy will be done and where we should expect changes in
floor levels. Specify elevation changes that might pose problems with door Cunction,etc.
♦ Supply and install in-floor wamung system in family room and kitchen (not budery). Infloor
heating should not go under any cabinets except below knee space under desk. Please provide
estimates for both hydronic and low-profile electric systems and provide detailed specs for
equipment with estimates.
♦ Supply and install Walker Zanger Pavio Antiqua-Walnut stone tiles throughout family room,
kitchen and budery. Available from RBC Tile&Stone,but if other distributors supply them for
less,any source is fine(although we would like to approve samples before final order is placed.)
Four sizes (8s8, 8s16, 16x16,& 16x24) —pattem attached. Fill holes in stone surface while
gcouting tiles in place. Grout color to be specified.
