CITY OF ORO*O <br />Municipal Offices <br />Post 0lfice lox 66 <br />Crystal lay, FIN 55323-0066 <br />ON-SITE SEWAGE TIREATKM <br />INSPECTION REPORT <br />Owner: TrirrlP ) Address: Iwo <br />Permit /'s: OU4 Dates: 7/r/Inn KZ Contractors: /cs !/Rt71r1111�1 <br />City ordinance nuamber 100 requires that each on-site sew"! treatment system in Orono be inspected on a regular b]sis. <br />The on-site sewage treatment system at the above address has been inspected and appears to fall into the category <br />checked below. <br />(This is A an existing system I ) new construction) <br />"CODE SYSTEM" -A systam which meets all the location, design, and construction standards of the current City <br />Codes, and which is operating satisfactorily by treating and disposing of the entire current sewage input <br />without discharging any pollutants into ground or surface waters. <br />'CONFORMING SYSTEFI"•A system which does not meet all the location, design, and construction standards of the <br />current City Codes but was installed according to the code in effect at the time 0 installation, and which <br />is operating satisfactorily by treating and disposing of the entire current sewage int( it without discharging <br />any pollutants into ground or surface waters. <br />"NON -CONFORMING SYSTEM"•A prohibited system; a system located within a designated 100 -wear floodplain; any <br />system which may or may not meet all the location, design, and construction standards of the current City <br />Codes and which is failing for any reason; and any system wits less than 3 feet of unsaturated soil or send <br />between the distribution device and the limiting soil characteristics. <br />(The limiting soil cho acteristic r hes or ( ) has not been identified at this time. If the limiting soil <br />characteristic has rot been identified, this classification may be subject to revision.) <br />Tank inspection Indicates: <br />5 Pumpouut not needed at this time. <br />6 Sothis accumulation in tanks indicates they should berammed out this year to help prevent future problems. <br />7 Solids accumulation in tanks is at a critical level. Tar+ks should be purged out as soon as possible. <br />E System is discharging to the surface. Tanks aunt be pupped out within Ga hours to eliminate surface discharge. <br />9 Inspection risers missing -tanks could not be inspected. Inspection risers (4" dim. pipe) must be installed in each <br />tank at next ptppout If tar* s have not been pupped out within the last three years, they should be pupped <br />out now. <br />00 Inspection pipe is located directly� over ��tank baffle (does not glue accurate meawrament of solids accumulation). <br />tf gyL�hin the last three years, they should be �uwtt�mae�aml. <br />Dr field inspection indicates: <br />Drainfield is dry, no surfacing evident. <br />117222 Some evidence of surfacing, not critical yet. <br />13 Drainfield is saturated and visibly discharging untreated effluent to the surface <br />immediately.
