At the regularly scheduled Septemb--r 28, 1995 meeting of the Board of Managers, the subject
permit application was reviewed along with the followu:g exhibits -
I. Permit application 95-212 received September 7, 1995.
2. Site survey map received September 7, I995.
3. Construction plans received September 7, 1995.
Action was taken approving your permit application pending receipt and staff approval of the
following exhibits:
1. Stormsewer outfall design.
2. A maintenance agreement for t!ie proposed catch basinimanliole, which specifies the
methods, schedule, and respcnsible parties for maintenance.
Please be advised that the project is not authorized until the above has been submitted and you
have been notified of permit issuance.
