Enclosed please find an updated Automatic Payment Plan Authorization. I had fraud on my old account
and need to switch payor/debiting accounts.
• Please cancel payment from account ending in #***6418.
• Payment is enclosed for the July 2022 payment; I was not sure the debiting/payor account could
be switched by due date.
• New authorization from payor/debiting account #***7691 is enclosed to commence the
following billing cycle.
• Please update records to list the owner as Sarah EM Kostial. I am the sole owner of this
property; you can confirm via the Hennepin County Property Records.
