Dear Mr. Minken:
I am sending you this letter to document my observatio s during two inspections of your septic
system earlier this year. My first inspection was perform d in early May at which time I observed
that the first septic tank manhole had been dug up ar�d a ew plastic riser was in place rather than
the concrete riser than had been there in the past. Th other two septic tanks appeared to be
unaltered from when the system was originally installed. The pump tank riser had stains running
down the riser, indicating that water was entering the ank from the riser. The drainfield was
discharging effluent from the last trench. At this time th system was non-compliant with current
City and State codes due to the surface discharge.
My second inspection was performed June 28th. At this ime I observed that the second and third
tanks now had new plastic risers. Some additional soil ha been placed over the drainfield to drain
surface water downslope. The drainfield was not disch ing effluent to the surface and therefore
was compliant with current City and State codes.

Dear Mr. Sullivan:

My third inspection was performed August 26, 1996. T e inspection revealed that the septic tank
risers now show no evidence of leaking, but the drainfield trenches are overloaded. The effluent in
the inspection riser on the last trench has a liquid level fi e inches below the ground surface. The
drainfield is not discharging to the surface at this time, ut has a high potential for failure. The
system is classified as compliant at this time.

The on-stte sewage treatment system at the above address has bee inspected and appears to fall into the category
checked below.

SYSTEM CONFORMITY (1-3):
1 "CODE SYSTEM"-A system which meets all the location, design, �and construction standards of the current City
Codes, and which is operating satisfactorily by trea ing nd disposing of the entire current sewage input
without discharging any pollutants into ground or su face waters.
2 "CONFORMING SYSTEM"-A system which does not meet all he l cation, design, and construction standards of the
current City Codes, but was installed according to t e c e in effect at the time of installation, and uhich
is operating satisfactorily by treating and disposing of he entire current sewage input without discharging
any pollutants into ground or surface waters.
3 "NON-CONFORMING SYSTEM"-A prohibited system; a system loc Ited uithin a designated 100-year floodplain; any
system Which may or may not meet all the location, desi n, and construction standards of the current City
Codes and uhich is failing for any reason; and any yste with less than 3 feet of unsaturated soil or sand
between the distribution device and the limiting soil cha .acteristics.

TANK CONDITION (5-10):

6� Solids accumulation in tanks indicates they should be pump ou this year to help prevent future problems.
Solids accumulation in tanks is at a critical level. Tanks sho ld be pumped out as soon as possibte.
8 System is discharging to the surface. Tanks must be pumped out uithin 48 hours to eliminate surface discharge.
9 Inspection risers missing-tanks could not be inspected. Inspec ion risers (4" dia. pipe) must be installed in each
