Sec.13.04 RIGHTS OF SLJB.TECTS OF D�TA
Subd. 1. Type of data. The righcs of individual on whom the data is stored or to be stored shall be as set forth in this section.
Subd.2. Information reqirired to be given individual. An individual asked to supply private or confidenrial data concerning himself shall
be informed of: (a)the purpose and intended use of the requested data wichin the collecting'state agency, poliacal subdivision,or statewide system;
(b)whether he may refuse o'r is legally required to supply the requested data;(c)any Irnown consequence arising from his supplying or refusing to supply
private or confidential data;and(d)the idenrity of other persons or enrides au[horized by state or federal law to receive the data. This requirement shail
not apply when an individual is asked to suppiy investieadve dara, pursuan[to secrion 13.82, subdivision 5, to a law enforcement officer.
The commissioner of revenue mav place the nodce reauired under this subdivision in the individual income tax or propertv tax refund
instructions instead of on those forms.
Subd. 3. Access to data by individual. Upon request to a responsible authoriry,an individual shall be informed whether he is the subject
of stored data on individuals, and wherher it is classified as public, private or confidendal. Upon his further request,an individual who is the subject
of stored private or public data on individuals shall be shown the data wichout any charge to him and, if he desires, shall be informed of the concent
and meaning of that data. After an individual has been shown the private data and infoaned of its meaning,the data need not be disclosed to hun for
six months thereafter unless a dispute or accion pursuant to this section is pending or addidonal data on the individual has been collected or created.
The responsible authoriry shall provide copies of the private or public data upon request by the individual subject of the data. The responsible authority
may require the requesting person to pay the actual cosu of making, certifying,and compiling the copies.
The responsible authority shall comply immediately, if possible, with any request made pursuant to this subdivision,or within five days of
the date of the request,excluding Saturdays,Sundays and legal holidays,if immediate compliance is no[possible. If he cannot comply with the request
within that time,he shall so inform the individual,and may have an addidonal five days within which to comply with the request,excluding Saturdays,
Sundays and legal holidays.
Subd.4. Procedure when data is not accurate or complete. An individual may contest the accuracy or completeness of public or private
data concerning himself. To exercise this right,an individual shall norify in wridng the responsible authoriry describing the nature of the disagreement.
The responsible authoriry shall within 30 days either: (a)correct the data found to be inaccurate or incomplete and attempt to nodfy past recipienu of
inaccurate or incomplete data, including recipients named by [he individual; or(b)notify the individual thac he believes the data to be correct. Data
in dispute shall be disclosed only if the individual's statement of disagreement is included with the disclosed data.
The determination of the responsible authority may be appealed pursuanc to the provisions of the administrative procedure act relaring to
contested cases.

CONSUMER INFORMATION SHEET '
This wood has been preserved by pressure treatment with an ,
EPA-registered pesticide containing chromated copper arsenate (CCA)
to protect it from termite attack and decay. Wood treated with CCA
should be used only where such protection is important .
CCA penetrates deeply into and remains in the pressure-treated wood
for a long time . Exposure to CCA may present certain hazards .
Therefore, the following precautions should be taken both when
handling the treated wood and in determining where to use or dispose
of the treated wood.
USE SITE PRECAUTION FOR CCA PRESSURE-TREATED WOOD
* Wood pressure-treated with CCA preservatives may be used inside
residences as long as all sawdust and construction debris are
cleaned up and disposed of after construction.
* Do not use treated wood under circumstances where the preservative
may become a component of food or animal feed. Examples of such
sites would be structures or containers for storing silage or food.
* Do not use treated wood for cutting-boards or countertops .
* Only treated wood that is visibly clean and free of surface
residue should be used for patios, decks and walkways .
* Do not use treated wood for construction of those portions of
beehives which may come into contact with the honey.
* Treated wood should not be used where it may come into direct or
indirect contact with public drinking water, except for uses
involving incidental contact such as docks and bridges .
HANDLING PRECAUTIONS FOR CCA PRESSURE-TREATED WOOD
* Dispose of treated wood by ordinary trash collection or burial .
Treated wood should not be burned in open fires or in stoves,
fireplaces or residential boilers because toxic chemicals may be
produced as part of the smoke and ashes . Treated wood from
commercial or industrial use (e .g. , construction sites) may be
burned only in commercial or industrial incinerators or boilers in
accordance with state and Federal regulations .
* Avoid frequent or prolonged inhalation of sawdust from treated
wood. When sawing and machining treated wood wear a dust mask.
Whenever possible, these operations should be performed outdoors to
avoid indoor accumulations of airborne sawdust from treated wood.
* When power-sawing and machining, wear goggles to protect eyes from
flying particles .
* After working with the wood, and before eating, drinking, and the
use of tobacco products, wash exposed areas thoroughly.
* If preservatives or sawdust accumulates on clothes, launder before

reuse . Wash work clothes separately from other household clothing. '
* Treated wood should be dry before handling.
