Conditions: Project to be completed as described in plans submitted to the
MCWD office on October 13, 2003 according to the provisions of
this permit.
• Properly install and maintain all required MCWD Rule B,
erosion control measures until the disturbed areas are
restabilized
• Silt fence must be orange in color
• When the site is restablilized and the MCWD staff has
performed a final inspection, all silt fences must be removed

During routine inspections by the Minnehaha Creek Watershed District (MCWD) staff at the above
mentioned location, the following violations of permit#03-525 were observed and recorded on

• Gravel driveway not effective—sediment being tracked into the street
Web Site: • Western corner of silt fence not trenched and small sediment piles lying around area
vwuw.minnehahacreek.org • Silt fence along road by driveway not trenched in/not effective
• Tube/pipe left behind site in buffer area and pallet lying on silt fence
The MCWD requires that the permit holder take the following action:
• Maintain and install silt fencing to MCWD specifications until disturbed soils are vegetated.
Board of Managers Install silt fences in the front of each site, on all downhill slopes and around stockpiles. Silt
fence must be trenched in 6 inches and installed according to instructions.
Pamela Blixt • Add gravel to driveway to avoid tracking sediment into the street
James Calkins • Trench in the western corner silt fence and mulch the area around the corner near the planted
trees (see drawing on inspection report)
Lance Fisher • Trench in silt fence at front of site by driveway
Susan Goetz • Remove tube/pipe and pallet from rear of site
Monica Gross To avoid formal enforcement action from the Minnehaha Creek Watershed District which may
include fees for additional inspections and legal costs, and the delivery of a Stop Work Order,
