Dear Mr. Waade:
As requested in your reply to our letter of February 12 , 1992, the
expiration date of Permit #91-6115 is hereby extended to November
30, 1992. All other terms and conditions of the permit shall
remain in full force and effect.
Please notify us when the project is complete, and submit any as-
built survey (if applicable) or photos of the project you may have
available.
Every attempt should be made to complete this project within the
timeframe authorized under this permit.
