MNG 790000 As you may be aware, the general permit authorizing the discharge of treated contaminated ground water shall expire on February 29, 1996. In order to continue the permitted activity beyond the expiration date of the general permit, you must submit a completed application for permit reissuance before the expiration of the existing permit. Please complete the enclosed application and submit the original to the Minnesota Pollution Control Agency (MPCA) at the following address by December 31, 1995. Minnesota Pollution Control Agency Water Quality Division Attn: Point Source Compliance Section 520 Lafayette Road St. Paul, Minnesota 55155 The permit application will not be processed unless it has been correctly signed and the required $85.00 application fee has been submitted with the application. According to Minn. R. 7001.0060, the application should be signed as follows: For a corporation, by a principal executive officer. For a public facility, by a principal executive officer or ranking official (for a city the mayor must sign). For a partnership or sole proprietorship, by a general partner or the proprietor, respectively. Application for permit reissuance is not a function which can be delegated. Applications signed by a consulting engineer or testing laboratory will not be accepted.
