require immediate attention. The permit holder is thereby required to:
• Repair and maintain all silt fence around the parameter of the site
• Install and maintain a rock/ gravel entrance to the site

Formal enforcement action to which a permittee may be subject for permit
violations includes issuance of a Stop Work Order, fees for additional staff

#01-366 were observed and recorded on July 26, 2002:
• Unapproved use of a non-gravel construction enterance contributing to
excessive sediment deposition and collection on Brown Rd.

Please note that entrance is considered temporary and access will be allowed on
the conditional basis. Conditions of use will change if MCWD feels that these
terms are not being met. Additionally, an on site meeting outlining these items was
conducted with Dan Saiko on July 31, 2002. MCWD will be required to increase
monitoring of this site to ensure that these conditions are being met, allowing for
the possibility of Rule J fees to be enforced.
