The permit holder is thereby required to: • Repair and maintain all silt fence around the parameter of the site • Install and maintain a rock/ gravel entrance to the site Board of Managers Pamela G.Blixt All violations should be corrected by Monday February 18, 2002. After this date a Minnehaha Creek Watershed District representative will James Calkins return to inspect the required repairs made to the construction site. Development, Long Lake ;mail: dmineminnehahacreek.org Dear Frank: kb Site: During a routine inspection by the Minnehaha Creek Watershed District (MCWD) Iww.minnehahacreek.org staff at the above mentioned location, the following violations of permit application #01-366 were observed and recorded on July 26, 2002: • Unapproved use of a non-gravel construction enterance contributing to excessive sediment deposition and collection on Brown Rd. ;oard of Managers • Lack of storm drain BMP's in association with new Bituminous surfaces on site 'amela G.BIM Based on reasonable concerns expressed by Dan Saiko, Project Manager, the ames Calkins MCWD has agreed to allow the temporary use of the construction entrance without ance Fisher adding a gravel pad, shown in green and labeled 'area of concern' on the attached Aonlca Gross site map, based on the following agreed conditions: icott Thomas • Use of entrance on an as needed basis until conditions allow for access to rear iichard Millar of building around the West side. At this point, use of temporary construction lobed Schroeder entrance will be completely closed off by silt fence and no longer will considered an acceptable entrance by the MCWD. • Daily scrape of sediment off sidewalk and street by means of bobcat or any necessary action required to remove sediment. Please note that entrance is considered temporary and access will be allowed on the conditional basis. Conditions of use will change if MCWD feels that these terms are not being met. Additionally, an on site meeting outlining these items was conducted with Dan Saiko on July 31, 2002. MCWD will be required to increase monitoring of this site to ensure that these conditions are being met, allowing for the possibility of Rule J fees to be enforced.
