AGREEMENT AND SWORN STATEMENT
The work for which this permit is issued shall be performed according to
the approved plans and specifications,applicable City approvals,and the
State Building Code. This permit is for only the work described and does
not grant permission for additional or related work which requires separate
permits. All provisions of laws and ordinances governing this type of work
shall be compied with whether or not specified herein.This permit will
expire and become null and void if construction authorized is not
commenced within 180 days of the date of issuance,or if construction is
suspended for a period of 180 days at any time after work has commenced.
The applicant is responsible for assuring all required inspections are
requested in conformance with the State Building Code.This permit may be
revoked at any time for due cause.

CITY OF ORONO—AUTOMATIC FIRE SPRINKLER/GENERAL PERMIT
(All permits must be approved by the Fire Marshall and/or Building Official)
1. Permits are required for all fire sprinkler installation and repair. All work shall be done by a licensed
fire sprinkler contractor. Two sets of plans, specification data sheets, fire flow tests and hydraulic
calculation sheets shall be submitted to the Orono Fire Marshal a minimum of 7 days before start of
work.
2. Permits can be applied for by mail at PO box 66, Orono, MN, 55323 or in person at,
2750 Kelly Parkway; Phone: (952)249-4600. Permits are also available online at:
www.ci.orono.mn.us
3. All systems shall be designed, installed and maintained to N.F.P.A.-13,N.F.P.A.-25, Minnesota State
Building Code, Minnesota State Fire Code and Standards. All attic systems are to be spaced at
maximum 130 square foot coverage. Plastic pipe will not be allowed at any time in attic spaces.
4. All equipment installed shall be U.L. or F.M. approved for fire protection service. Water flow test
shall be completed on all new systems. Water flow test reports shall be submitted to the Orono Fire
Marshal along with plan submittal.
5. Yard or wall post indicator valves are required. All indicating and control values installed shall be
provided with tamper protection. On dry systems,the control valve to service the pressure switch (air)
shall be supervised.
6. Inspectors test valve shall be installed on each floor level or zone of system. All systems shall have
main drain and inspectors test valves piped to the outside.
7. No water is to be introduced into the sprinkler system until main has been thoroughly flushed. 2004
air test and flushing shall be witnessed by the City of Orono Water Department.
8. An approved audible/visual device wired to main flow switch shall be installed above F.D.
connection and in areas normally occupied by tenants.
9. EXISTING SYSTEMS: If any changes in the hydraulically most demanding area, change in
occupancy classification or an addition of 20 or more heads, hydraulic calculations and flow test will
be required.
10. The Orono Fire Marshal shall witness all final flow or trip tests. Appointments can be made by
calling the Fire Marshall at(952)473-9701, 8:00-4:00 p.m. Monday-Friday. 24-hours notice is
requested.
11. When required, (30 foot building height)stairwell standpipe's shall have 2 'Agate valves installed
with 2 V2 x 1 '/2 caps.

SCOPE: The plans and specifications describe the water service and building sewer connections for the
referenced facility. The review is limited to the design of this particular project only, and does not cover the
plumbing within the building. The review is based upon the supposition that the data on which the design is
based are correct, and that necessary legal authority has been obtained to construct the project. The
responsibility for the design of structural features and the efficiency of equipment must be taken by the project
designer. Approval is contingent upon satisfactory disposition of any requirements included in this report.
Special care should be taken to insure that the material and installation are in accordance with the provisions of
the Minnesota Plumbing Code and applicable licensing requirements.
A set of the identified plans and specifications is being returned to CDR Excavating Inc.
INSPECTIONS: All plumbing installations, including water service and building sewers,must be tested and
inspected in accordance with the requirements of the Minnesota Plumbing Code. It is the responsibility of the
contractor/installer to notify the Minnesota Department of Labor and Industry when an installation for a state
contract job, licensed facility, or project in an area where there is no local administrative authority is ready for an
inspection and test. To schedule inspections, contact the state plumbing standards representative for your region,
or call Jim Peterson at 651/284-5889.
REQUIREMENT(S):
1. The use of a 4-inch high density polyethylene(HDPE)pipe complying with AWWA C906 has been
proposed for the water service installation. The pipe is to be installed by directional boring. Note that
AWWA Standard C906 polyethylene pipe is not an approved material listed in the Minnesota Plumbing
Code,nor is directional boring an approved installation method listed in the Minnesota Plumbing Code.
However,this pipe material and method may be used if approved by the city of Orono building official,Mr.
Lyle Edward Oman, as an alternate material or method under Minnesota Rules,part 4715.0330.
If this material and method are not approved by Mr. Oman,verify that the material used for the water
service installation complies with Minnesota Rules,part 4715.0510.
2. The water service installation must be disinfected in accordance with Minnesota Rules,part 4715.2250.
3. The water service installation must be tested in accordance with Minnesota Rules,part 4715.2820.
NOTE(S):
1. The scope of this project consists of the installation of a new municipal water service line to an existing
building.

2. This facility is served by an existing individual sewage treatment system.
Prior to the installation of additional plumbing,not covered by this review,plans and specifications must be
submitted to and approved by this office.
Authorization for construction in accordance with the approved plans may be withdrawn if construction is not
undertaken within a period of two years. The fact that the plans have been approved does not necessarily mean
that recommendations or requirements for change will not be made at some later time when changed conditions,
additional information, or advanced knowledge make improvements necessary.
