This permit is not transferable without District approval, and is valid to the date of
Susan Goetz expiration. No activity is authorized beyond the expiration date. If the permittee
Richard Miller requires more time to complete the project, an application for renewal of the
Ethel Smith permit must be received by the District at least 30 days before expiration.
SoottThomas The applicant is responsible for compliance with all District Rules and for the
action of their representatives, contractors, and employees.
Conditions: Project to be completed as described in plans submitted to the
MCWD office on August 3, 2004 according to the provisions of
this permit.

Recommendation:
Approval. Permit should not be issued until the following conditions are met:
1. Reimbursement of mailing costs
2. Reimbursement of Rule] fees
3. Submit a declaration for the preservation of the wetland buffer which must be executed
and recorded
Background:
The applicant is proposing to construct and improve a driveway access to single-family home
sites at the address cited above in the City of Orono. There are two delineated wetlands on site.
One large type 3/6/7 wetland greater that 80 acres in size occupies over 40 acres of the 50 acre
parcel and one type 3/6 wetland less then one acre exists to the East of the existing access road.
There is an existing entrance road through out lot A which will be improved and expanded into
the upland to access the two platted housing sites. Wetlands will not be further impacted through
filling activities involving this project. Permits will allow for construction of the driveway to
access the single-family home sites and the establishment of wetland buffers.
Rule B:
Erosion control consisting of silt fence has been provided throughout the site in addition to an
existing crushed rock driveway which has been designated for access to the site
Rule D:
As stated above, no filling activities will occur within the wetland as a result of this project. The
applicant has agreed to install two new 12 inch culverts under the existing access road to allow
for wildlife crossing and maintain a hydrologic connection between the 80 acre wetland and a
less then one acre wetland to the East. The applicant has proposed wetland buffers 16.5 feet and
35 feet in accordance with Rule D.
