State Building Code. This permit is for only the work described and does
not grant permission for additional or related work which requires separate
permits. All provisions of laws and ordinances governing this type of work
shall be compied with whether or not specified herein.This permit will
expire and become null and void if construction authorized is not
commenced within 180 days of the date of issuance,or if construction is
suspended for a period of 180 days at any time after work has commenced.

Visual Observation Report From The Ground:
- American Basswood: DBH 38", 34" approximately 95'tall. Two large Basswood trees growing
together in the back yard one is leaning towards the fence and lake and the other is leaning
towards the house.They have visible decay in the trunk wood with large split sections that are
hollow in the center.There are large decay cavities in the upper canopy that also appear to be
hollow in the center.The support system that was installed in the tree has failed due to soft
tissue wood and possible decay in the upper canopy.There are grade issues with both trees,the
root flare is not exposed, and appears to have girdling/binding roots and decay at the base of
the trees. Leaves appear to be scorched throughout the canopy.
- White Ash: DBH 14",approximately 30'tall.Tree is growing on a 45 degree angle over the lake.
The root system is up rooting and causing ground heaving and shifting of the rock sea wall.The
tree appears to be healthy although the root system appears weak.
