Based on our inspection at the above mentioned location,the following was observed and recorded on April 19, 2012: • Working without a MCWD Erosion Control permit • Soil leaving construction site MCWD requests that the following actions are taken: • Install/repair construction entrance immediately with either crushed rock or frequent street sweeping to prevent soil from leaving site • The MCWD Board of Managers conditionally approved the above mentioned permit. Therefore, the following conditions must be met in order to issue the permit(please see original approval letter attached): • Reimbursement of mailing costs in the amount of$59.40 • Reimbursement of engineering review fees in the amount of$679.40 • Submit a recorded copy of a declaration for stormwater facility maintenance and wetland buffers.
