INSPECTION REPORT
DATE: April 23, 2012
TO: Adam Ginkel,Plowe Engineering
FROM: Mary Hammes, District Representative
RE: Permit# 10-143: 2535 Shadywood Rd,Excelsior
The Minnehaha Creek Watershed District(MCWD) has inspected your site for compliance with the
MCWD permit requirements. Based on our inspection at the above mentioned location,the following
was observed and recorded on April 19, 2012:
• Working without a MCWD Erosion Control permit
• Soil leaving construction site
MCWD requests that the following actions are taken:
• Install/repair construction entrance immediately with either crushed rock or frequent street
sweeping to prevent soil from leaving site
• The MCWD Board of Managers conditionally approved the above mentioned permit. Therefore,
the following conditions must be met in order to issue the permit(please see original approval
letter attached):
• Reimbursement of mailing costs in the amount of$59.40
• Reimbursement of engineering review fees in the amount of$679.40
• Submit a recorded copy of a declaration for stormwater facility maintenance and wetland
buffers.
Please let this serve as a reminder that immediate and specific attention may be needed to improve
erosion and sediment controls on your project. Application materials can be found on our website at

To avoid formal enforcement action from the Minnehaha Creek Watershed District,please implement
